Dear Ms. Lane:
Fidelis, LLC (Fidelis) is pleased to submit the following report of the asbestos survey conducted at the Birmingham VA Medical Center in Birmingham, AL. This asbestos survey includes samples taken over multiple levels of the medical center, which included mechanical spaces, crawl spaces, and workspaces (offices). All samples were taken with the consent and at the direction of the COR (Sandy Davis). Samples were taken from the sub-basement, basement, ground floor, 2nd floor, 5th floor, 9th floor, 10th floor, 11th floor, and in Research (floors 3, 4, 5, and labeled roof/mechanical space on 6). Fidelis reviewed areas and visually inspected the areas that were sampled in previous reports to determine the condition of the asbestos containing materials and if they were still present. This sampling event was conducted in accordance with the scope of work issued by the Department of Veterans Affairs, dated September 23, 2013.

1 PURPOSE AND SCOPE OF SERVICES
This survey is intended for the sole purpose of identifying suspect materials that may contain asbestos at the Birmingham VA Medical Center. A visual inspection of previously sampled asbestos containing materials in the Main Building, Research Wing and the Z Wing of the Birmingham VA Medical Center at 700 South 19th Street. The purpose of this document is to compile, update and expand the original asbestos management plan and other previous re-inspection documents. To accomplish this, areas, which have been completely abated and homogeneous areas that no longer remain have been eliminated from the asbestos containing material inventory and the correct information concerning the location and quantity have been updated. Each homogeneous area of asbestos containing material, which currently remains, has been assigned a revised number, photographed, described and quantified. Revised estimates of removal costs are presented for each of the asbestos containing material homogeneous areas are included within this report.
Specifically, Fidelis provided the following services:
1. Documented readily apparent and accessible friable and non-friable asbestos-containing materials (ACMs) within specified areas of the subject site;
2. Conducted a visual inspection of specific areas of the subject site to identify suspect
ACMs by an accredited asbestos inspector;
3. Sampled suspect materials, as required;
4. Analyzed those materials suspected of containing asbestos by Polarized Light
Microscopy (PLM) and dispersion staining techniques in accordance with the
Environmental Protection Agency’s (EPA) Bulk Analysis Method 600/R-93/116;
5. Provided a description of those materials identified in the structures as containing asbestos;
6. Prepared a written report documenting the following information:
• Confirmation for the presence of ACMs in the building at specific sampling locations
• Estimation of quantities of identified ACMs

• Options and recommendations for remediation alternatives related to existing
ACMs
• Production of drawings showing the sample locations as well as previously identified asbestos-containing materials
It should be noted that, if any renovation work has been performed at the subject site, suspect ACMs including floor tile, ceiling tile, pipe insulation, plaster, various mastics, and miscellaneous other materials may have been concealed under carpet, above suspended ceilings, or in numerous other locations. It is also not uncommon to find multiple layers of floor tiles and multiple layers of roofing materials that were not visible during this survey. Fidelis recommends that if concealed suspect ACMs are discovered they be sampled and analyzed for asbestos content prior to any renovation or demolition activities.

2 PROJECT CHARACTERISTICS
The Birmingham VA Medical Center is made up of 14 levels located at 700 South 19th Street in Birmingham, AL. The building was originally constructed in its current location in 1953 with numerous additions constructed over the years. The sub-basement, 10th and 11th floors are made up of mechanical spaces. The ground floor through the 9th floor include a mixture of workspaces, examination rooms, patient rooms, and mechanical spaces. The basement is comprised of a mixture of mechanical spaces, crawl spaces, and offices. There is also a research department made up of laboratories, storage areas, and mechanical spaces. The Z Wing is made up of patient areas, mechanical rooms, labs, offices, and a wood shop.
Fidelis conducted an asbestos survey under the supervision of the Safety Office and collected samples from the sub-basement, basement, ground floor, 2nd floor, 5th floor, 9th floor, 10th floor, 11th floor, and within the Research Wing. Fidelis collected 290 samples from 72 homogeneous locations throughout the Birmingham VA Medical Center’s Main Building and Research Wing (with 7 of these homogeneous areas testing positive for asbestos). Fidelis also conducted a visual inspection of any asbestos-containing materials that were discovered during previous reports. Any areas that have been abated since the last report (the re-inspection was completed in 2010) have been removed from this report. Fidelis conducted this visual inspection through the three main areas of the Birmingham VA Medical Center: the Main Building, the Research Wing, and the Z Wing. Photographs of all areas sampled and visually inspected homogeneous areas have been included within this report.

3 ASBESTOS SAMPLING METHODOLOGY
Visual inspections were performed throughout the Medical Center, specifically in locations that were previously identified in prior asbestos surveys, including the Main Building, the Z Wing, and Research Wing. Suspect materials were sampled by physically removing a section from the area of concern for analysis. Where applicable, suspended ceiling tiles were lifted to inspect the space above. Bulk samples were then placed into individual sample containers for transport to a qualified laboratory for analysis.
All samples were individually identified by a unique sample identification number. The suspect ACMs were numbered and correspond to the sample numbers listed in the PLM Analysis Summary located in Appendix D of this report. Photographs of positive sample locations are included in Appendix A of this report. Photographs of all samples collected during the 2013 sampling event are included in Appendix C.

For example, sample SB6-FP1-1 is the 6th sample collected from the VA. It was collected from the sub-basement. The sample was the 1st fireproofing that was encountered at the site.

4 HOMOGENEOUS MATERIAL ASSESSMENT
FACTORS
4.1 Homogeneous Areas
Before collecting samples, distinct homogeneous sampling areas and specific sampling sites were determined. A homogeneous sampling area can be described as any material of the same type. In other words, a homogeneous sampling area is uniform throughout in structure or makeup and appearance (color, texture, and/or date of construction/application). Homogeneous areas can be further classified as surfacing material, thermal system insulation, and miscellaneous materials. Samples were collected of each specific homogeneous material identified.
Photographs of the positive homogeneous areas are included in the Homogeneous Area descriptions in Appendix A. Pictures of all sample locations associated with the 2013 sampling event are included in Appendix C.
4.2 Asbestos Hazard Assessment Factors
Friability was determined each time a suspect ACM was sampled. Friable means that the material can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Also, materials thought to be and typically classified as non-friable may receive a reclassification of friable if the material is damaged. Friable ACM has been determined by the EPA and Occupational Safety & Health Administration (OSHA) to be more hazardous than non-friable ACM. Friable material can create a situation of airborne asbestos fibers more readily than non-friable materials. In assessing the fiber release potential, only the current conditions of all ACM identified were noted.
Only materials readily accessible within the specific project area were evaluated.
Materials that were hidden and/or not accessible were not evaluated as part of this survey. These hidden and/or inaccessible materials should be analyzed if they are located during any renovation or demolition activities. Materials visibly identified as non-asbestos (fiberglass, foam rubber, wood, etc.) were not sampled.

5 LABORATORY ANALYSIS
All suspect bulk asbestos samples were analyzed by a qualified laboratory using Polarized Light Microscopy with dispersion staining techniques. PLM and dispersion staining techniques were performed according to the EPA's Bulk Analysis Method EPA 600/R-93/116 in an attempt to ascertain asbestos content in those samples analyzed.
These techniques require that a part of the bulk sample be subjected to an oil of specific refractive index once mounted on a slide. This prepared slide is then subjected to a number of optical tests. Each type of asbestos displays unique characteristics when subjected to these tests. Percentages of asbestos content are then determined by visual estimation.
Any material containing greater than one percent of any type of asbestos is classified as ACM by the EPA and OSHA. According to the EPA, OSHA, United Stated Department of Transportation (USDOT), and other federal, state, and/or local government agencies, any material classified as ACM must be handled accordingly.

6 RESULTS BY FLOOR
The following table describes the sample number, sample description and location, asbestos content, friability, and condition for each of the 290 samples collected from 72 homogeneous areas (not including multiple layers) from October 7, 2013 through November 19, 2013. Friability is described if the sample was shown to contain asbestos and is described based on the current condition of the material. Photographs of positive sample locations are included in Appendix A and a copy of the laboratory testing and analysis data is included in Appendix E of the report.
The sampling locations are provided in the table in Appendix D and are shown on the drawings in Appendix B. During laboratory sample preparation, if multiple layers of an individual sample were encountered, the laboratory analyzed each layer separately.
The condition of the material was determined at the time of sampling. Bold type indicates an asbestos-containing sample.
6.1 Sub-Basement
Samples were collected from the sub-basement on October 7, 2013, as directed by the Safety Office. The sub-basement was divided into two sections. Twenty-nine samples from 10 homogeneous areas were collected from the southern portion of the sub-basement and 27 samples from 12 homogeneous areas were collected from the northern portion. Sampling results are discussed below.
6.1.1 Asbestos Sampling Results for the Sub-basement
Of the 22 homogeneous areas, only one was positive. This sample was a white, cementitious pipe insulation that appeared to be a patch. This material was observed in the northern portion of the sub-basement and was in poor condition.
6.1.2 Estimation of Quantities for the Sub-basement
The only asbestos-containing material encountered in the sub-basement was a patch on a pipe run. It is estimated that there are approximately four linear feet of this material.
6.2 Basement
Samples were collected from the basement on October 8, 2013. Thirty samples were collected from 11 homogeneous areas. Sampling results are discussed below.
6.2.1 Asbestos Sampling Results for the Basement
Of 11 homogeneous areas, one was positive. This material was tan 9” x 9” floor tile and black mastic located in an electrical closet (SPB103A-BH). This material was observed to be in fair condition.

6.2.2 Estimation of Quantities for the Basement
It is estimated that approximately 15 square feet of the floor tile is located in this area (SPB103A-BH). It should be noted that there were other areas of floor tile that were not sampled. It should be assumed that other areas of floor tile are positive until sampling proves otherwise.
6.3 Ground Floor
Samples were collected from the ground floor on November 4, 2013. Thirty-one samples from 11 homogeneous areas were collected. Sampling results are discussed below.
6.3.1 Asbestos Sampling Results for the Ground Floor
Of 11 homogeneous areas, two were positive. The first material is grey fireproofing that was observed in the electro-mechanical room G415. This material was observed to be in fair condition.
The second material was sealant located on the pipe joints of purple-painted pipe insulation in the mechanical room near the canteen (M-R-1). This material was observed to be in poor condition.
6.3.2 Estimation of Quantities for the Ground Floor
It is estimated that there were 400 square feet of fireproofing in room G415 (SPG415- BH). We also estimated 300 linear feet of purple painted pipe insulation in the mechanical room (M-R-1).
6.4 Second Floor
Samples were collected from the second floor on November 19, 2013. Twenty-one samples were collected from seven homogeneous areas. Sampling results are discussed below.
6.4.1 Asbestos Sampling Results for the Second Floor
None of the samples contained asbestos.
6.4.2 Estimation of Quantities for the Second Floor
None of the samples contained asbestos.

6.5 Fifth Floor
Samples were collected from the fifth floor on November 7, 2013. Nineteen samples from six homogeneous areas were collected. Sampling results are discussed below.
6.5.1 Asbestos Sampling Results for the Fifth Floor
Of six homogeneous areas, one was positive. This material was 12” x 12” floor tile with black mastic located in a chase next to the service elevator. This material was observed to be in fair condition.
6.5.2 Estimation of Quantities for the Fifth Floor
It is estimated that approximately 10 square feet of the floor tile in this area. It should be noted that there were other areas of floor tile that were not sampled. It should be assumed that other areas of floor tile are positive until sampling proves otherwise.
6.6 Ninth Floor
Samples were collected from the ninth floor on November 7, 2013. Twenty-one samples were collected from eight homogeneous areas. Sampling results are discussed below.
6.6.1 Asbestos Sampling Results for the Ninth Floor
Of the eight homogeneous areas, one was positive. This material is the black wall mastic that is reportedly located on all exterior walls. This material was in good condition.
6.6.2 Estimation of Quantities for the Ninth Floor
It was estimated that 50 square feet on the black wall mastic was present in the mechanical closet inspected. It is our understanding that this material exists on most of the exterior walls in the building.
6.7 Tenth Floor
Samples were collected from the tenth floor on November 7, 2013. Seven samples were collected from two homogeneous areas. Sampling results are discussed below.
6.7.1 Asbestos Sampling Results for the Tenth Floor
Of the two homogeneous areas, one was positive. This material is the 12” x 12” brown floor tile and black mastic located inside of a closet (SP10THA-BH). This material was in good condition.

6.7.2 Estimation of Quantities for the Tenth Floor
It is estimated that approximately 15 square feet of the floor tile in this area (SP10THA- BH). It should be noted that there were other areas of floor tile that were not sampled (on other levels of the medical center). It should be assumed that other areas of floor tile are positive until sampling proves otherwise.
6.8 Eleventh Floor
Samples were collected from the eleventh floor on November 7, 2013. Fifteen samples were collected from five homogeneous areas. Sampling results are discussed below.
6.8.1 Asbestos Sampling Results for the Eleventh Floor
None of the samples contained asbestos.
6.8.2 Estimation of Quantities for the Eleventh Floor
None of the samples contained asbestos.

7 CURRENT REGULATIONS
It is our understanding that this survey was conducted to determine asbestos-containing material locations for future renovations at the Birmingham VA Medical Center.
Current regulations require removal of ACMs from facilities that conduct activities that can damage or disturb ACMs. These activities include renovation or demolition work.
When disturbing in-place asbestos, OSHA asbestos regulations (29 Code of Federal Regulations [CFR] Parts 1910 and 1926) and the NESHAPS asbestos regulations (40 CFR Part 61, Subpart M), should be followed.
NESHAPS (40 CFR Part M, Amended 20 November 1990) regulates the removal and disposal of ACM. This regulation separates ACM into three categories based on the material’s friability and/or physical condition:
• Regulated ACM (RACM)
• Category I nonfriable ACM
• Category II nonfriable ACM
Category I nonfriable ACM includes gaskets, valve packings, resilient vinyl floor covering (floor tile and linoleum), and asphalt roofing material containing more than one percent asbestos by volume. Category II nonfriable ACM is any other nonfriable material with more than one percent asbestos by volume. An example of a Category II nonfriable material would be asbestos-cement board (transite). Regulated ACM includes:
• Friable ACM
• Category I nonfriable ACM that has become friable
• Category I nonfriable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading
• Category II nonfriable ACM that has a high probability of becoming friable or can be rendered friable by the forces expected to act on the material during demolition activities
By EPA definition, regulated ACMs must be abated by a licensed abatement contractor prior to any activities which will disturb the materials (such as renovation or demolition) are to occur.

8 CONCLUSIONS AND RECOMMENDATIONS
Based on observations and samples collected at specific locations during the asbestos survey conducted by Fidelis at the Birmingham VA Medical Center at 700 South 19th Street on October 7, 2013 through November 19, 2013, the wrap of woven paper with fibers over mineral wool insulation on a pipe elbow (Sub-basement 2), floor tile with associated black mastic (basement storage closet – SPB103A-BH), grey fire proofing in SPG415-BH, woven paper with aluminum backing over mineral wool on purple pipe run (located in M-R-1), 12” x 12” floor tile with black mastic (5th floor chase next to service elevator), wall mastic in 9th floor electrical closet (assumed on all exterior walls), and 12” x 12” brown floor tile with black mastic in SP10THA-BH. If these materials will be disturbed during demolition or renovation activities, they will require abatement.
Additional materials identified during previous sampling events were observed for condition on March 6, 2014.
Fidelis also recommends that if suspect materials that were hidden and/or not accessible during this sampling event are discovered during any renovation or demolition activities at the subject site, the suspect materials should be analyzed for asbestos content.
If the materials are to be left in place, an Operations and Maintenance (O&M) Plan should be written (in accordance with Title 29 of the Code of Federal Regulations, Part 1910.1001, Subpart K (29 CFR §1910.1001, Subpart K) and maintained on site to manage the asbestos-containing materials in place.

9 ASSUMPTIONS AND LIMITATIONS
Fidelis notes that this Asbestos Survey was limited to the collection of bulk samples from readily accessible locations. Laboratory analysis utilizing PLM along with dispersion staining techniques was used to determine asbestos content in those samples taken.
The results, findings, observations, and recommendations expressed in this report are based only on conditions that were observed during our inspection of the site, conducted on October 7, 2013, through November 19, 2013, and during our visual inspection of previously identified materials, conducted on March 6, 2014. Although care has been taken by Fidelis in compiling and checking the information contained in the report to verify that it is current and accurate, we disclaim any and all liability for any errors or omissions or inaccuracies. The recommendations provided in this report do not constitute legal and/or medical advice. It is further understood that Fidelis makes no representations or warranties of any kind, including but not limited to, the warranties of fitness for a particular purpose or merchantability, nor are such representations or warranties to be implied with respect to the data furnished. We assume no responsibility with respect to customers, its employees, client's or customers' use thereof. Fidelis, their representative, and this report make no representation and/or assumptions as to past conditions or future occurrences at this site. The information provided herein applies only to the subject property as it existed during our site visit.

The purpose of this asbestos survey is to identify suspect materials in specific locations that may contain asbestos at the subject site. An Asbestos Survey is typically not sufficient to estimate costs of abatement or to obtain access to areas that may not be exposed until work begins.
