Subject: Limited Hazardous Materials Survey Building 143, 4th Floor - Central Elevator Lobby and H Wing 1970 Roanoke Boulevard Salem, Virginia H&P Project 20190443
Mr. Holihan, Hurt & Proffitt, Inc. (H&P) was requested to conduct sampling of suspect asbestos containing materials (ACM) and lead based painted building materials and to identify potential mercury and PCB containing components in Building 143 on the 4th Floor at Salem Veterans Affairs Medical Center in Salem, Virginia.
The sampling and review of the Central Elevator Lobby area was performed by H&P representatives Roy Wriston (Virginia Asbestos Inspector License number is 3303 003648) and Mary Beth Wriston (Virginia Asbestos Inspector License number is 3303 003466) on May 13-14, 2019. At your request, H&P has also incorporated previous findings from our survey and review of the adjoining H Wing into this report.
The subject site for this report is the 4th floor of Building 143 which houses patient rooms and associated medical support facilities and comprises two separate wings (H and J) and a Central Lobby section which adjoins the wings and houses the elevators that serve the floor. The survey data for the Central Lobby, H Wing and Room 117 (Storage Room) of the J Wing are included in this report.
1.0 Asbestos Sampling Procedures and Findings
The intent of the asbestos sampling was to identify asbestos containing materials (ACM) in the affected portions of the building within the Central Lobby, H Wing, and Room 117 of J Wing that could be disturbed by planned renovation work. Materials sampled included fireproofing, floor tiles and associated mastics, drywall and joint compound, ceiling plasters, ceiling tiles, piping and duct insulations and mastics and vapor barriers located below ceramic tile walls and flooring in the shower room. We note that this was a limited invasive survey of accessible materials that could be sampled without damaging surfaces and systems components. The survey areas were occupied during our site visits which limited our ability to access and sample all potential concealed materials. If materials are encountered that were not represented during this limited survey; they must be sampled and analyzed for asbestos prior to disturbance.

The survey included collection of one hundred and seventy four (174) individual samples and layers of suspect asbestos containing materials (ACMs) that were submitted for analysis of asbestos. The suspect ACM samples were submitted to SanAir Technologies Laboratory an NVLAP accredited and Virginia licensed asbestos laboratory in Powhatan, Virginia, for analysis by Polarized Light Microscopy (PLM) following EPA Method 600/R-93/116. The analytical results are shown in the following table. Copies of the laboratory Asbestos Analysis Report and Chain of Custody forms are included as an attachment to this report.

2.0 Sample Analysis Results and Recommendations
Based on the results of analysis, asbestos was not detected in the materials sampled during this survey.
If during renovation/demolition, work is performed that will impact suspect asbestos containing materials that have not been sampled, it is recommended that these materials be sampled by a Virginia licensed asbestos inspector prior to disturbance.
2.1 Presumed Asbestos Containing Materials
During the conduct of this survey, sampling was limited to those materials which were within the areas designated by the client, which were safely accessible and which were able to be sampled without damaging systems or structures. As such, some materials that were not accessible or other not sampled should be presumed to be positive, unless sampling is conducted and shown to be negative. Such presumed asbestos containing materials (PACMS) observed at the site include, but are not limited to:
1. Fire door interiors, if present;
2. Electrical panel backing;
3. Concealed (above ceilings or within wall chases) piping or ducting insulation, if present.
4. The sample of flooring materials collected from Room 117 in the adjoining J Wing was of the ceramic flooring and underlying materials. During our surveys, this was the only ceramic floor sampled due to the other rooms being occupied and/or the potential to damage the floors in a manner that could allow for water intrusion and damage as in the case of bath and shower rooms that were still in use. Restroom and Shower Room locations within the wings and the lobby area have ceramic tile flooring with suspected vapor barriers underlying them. These other ceramic tile floors were not sampled during our surveys and as such, should be considered to have suspect asbestos containing materials underlying them. Therefore, once these rooms are no longer in use, they should be sampled for asbestos by penetration the tile and removing cores of the underlying materials prior to disturbance.
If during renovation/demolition, work is performed that will impact suspect asbestos containing materials that have not been sampled, it is recommended that these materials be sampled by a Virginia licensed asbestos inspector prior to disturbance.
If asbestos abatement activities are performed within the building, all work should be performed by a Virginia licensed asbestos abatement contractor.
3.0 EPA/NESHAP Regulations for Asbestos Containing Materials
The U.S. Environmental Protection Agency promulgated the National Emission Standards for Hazardous Air Pollutants (NESHAP) [40 CFR Part 61], which addresses the application, removal, and disposal of asbestos-containing materials (ACM). Under NESHAP the following categories are defined for asbestos-containing materials:
Friable - When dry, can be crumbled, pulverized, or reduced to powder by hand pressure.
Non-friable - When dry, cannot be crumbled, pulverized, or reduced to powder by hand pressure.
Category I Non-friable ACM - Packings, gaskets, resilient floor coverings, and asphalt roofing products containing more than 1% asbestos.
Category II Non-friable ACM – Any material, excluding Category I Non-friable ACM, which contains more than 1% asbestos.
Regulated Asbestos Containing Material (RACM) – One of the following:
1. Friable ACM
2. Category I Non-friable ACM that has become friable.
3. Category I Non-friable ACM that will be or has been subjected to sanding, grinding, cutting, or abrading.
4. Category II Non-friable ACM that has a high probability of becoming, or has become, friable by the forces expected to act on the material in the course of demolition or renovation operations.
Under NESHAP, the following actions are required:
1. Prior to the commencement of demolition or renovation activities, the building owner must inspect the affected facility or part of the facility where the demolition or renovation activities will occur for the presence of asbestos.
2. Remove all RACM from the facility before any activity begins that would break up, dislodge, or similarly disturb the material or preclude access for subsequent removal.
3. RACM need not be removed if:
a) It is Category I non-friable ACM that is not in poor condition.
b) It is on a facility component that is encased in concrete or other similar material and is adequately wet whenever exposed.
c) It was not accessible for testing and was therefore not discovered until after demolition began and because of the demolition the material cannot be safely removed.
d) It is Category II non-friable ACM and the probability is low that the material will become crumbled, pulverized, or reduced to powder during demolition.
Note: Based on our experience, we recommend that all ACM be removed prior to renovation or demolition due to the fact that disturbance can damage them, create potential exposure to workers and occupants and make them friable. This recommendation is made as a best practice to reduce potential exposure and limit liability.
4.0 OSHA
The Occupational Safety and Health Administration (OSHA) regulates employee exposure to asbestos under 29 CFR 1926.1101 and 29 CFR 1926.1001. Work associated with known or suspect asbestos containing materials (ACM), including trace (<1%) levels of asbestos, must be conducted according to these regulations in addition to the noted EPA regulations where applicable.
If the materials that have been identified as asbestos containing will be disturbed or are in poor condition, it is recommended that the asbestos be removed by an appropriately trained and licensed asbestos abatement contractor in accordance with Virginia, EPA and OSHA regulations.
If work is performed that will impact suspect materials that have not been sampled, it is recommended that they be sampled by a Virginia licensed Asbestos Inspector prior to disturbance. We note that during renovation or demolition activities, it is not uncommon to reveal materials that were not accessible during the survey or their presence could not otherwise be known.
5.0 Lead Based Paint Testing Procedures
Testing for lead based paint was performed by sampling representative materials and components using a Niton XLp-300A X-Ray Fluorescence (XRF) Lead Paint Analyzer. The XRF contains a small radioactive source (Cadmium 109) which emits radiation when exposed to a surface. If the paint contains lead, the radiation will stimulate the lead atoms to emit characteristic x-rays, which are sensed by a detector in the unit. The XRF then converts these signals to a final reading in milligrams of lead per square centimeter of surface area tested (mg/cm2). Calibration of the Analyzer was conducted in accordance with the manufacturer’s instructions using standard reference materials with different lead concentrations. The Analyzer must indicate readings within the acceptable calibration range for the average of three readings.
If the average of the three readings is within the established tolerance, the unit is working properly.
Calibration checks were performed prior to and at the end of the testing. The XRF is able to accurately detect as little as 0.1 mg/cm2 of lead. The regulatory concentration for identifying paint film or coatings as lead based is 1.0 mg/cm², the concentration that was used as the action level for this survey. Where components tested below the designated action level of 1.0 mg/cm² for classifying the material as lead based paint; it is important to note that any painted or coated surface may contain concentrations of lead, which when disturbed, could generate lead in dust greater than the action level exposure concentration of 30 ug/m3 established by the OSHA “Lead Exposure in Construction Rule” (29 CFR 1926.62).
5.1 Lead Based Paint Testing Results
A total of one hundred sixty six (166) XRF readings, including calibrations, were collected from painted or surface coated building materials. Tested materials and components included:
Doors and door casings;
Walls;
Floors;
Windows;
Columns;
Ceilings;
Panel boxes;
Fire line;
Stair railing, riser and stringer;
Temperature pad.
5.2 Lead Based Paint Findings
The purpose of the screening survey was to identify painted surfaces and surface coatings that may contain lead at concentrations above the comparative standard for the identification of lead based paint.
Based on the results of XRF testing, lead based paint was not identified in the building materials tested.
6.0 Regulatory Compliance for Lead
H&P recommends that the contractor adhere to all applicable EPA regulations regarding disposal requirements and OSHA regulations for employee exposure.
6.1 OSHA Regulations
Where components tested below the designated action level of 1.0 mg/cm² for classifying the material as lead based paint (LBP), it is important to note that any painted or coated surface may contain concentrations of lead, which when disturbed, could generate lead in dust greater than the action level exposure concentration of 30 ug/m3 established by the OSHA “Lead Exposure in Construction Rule” (29 CFR 1926.62). The OSHA standard gives no guidance on acceptable levels of lead in coatings at which no exposure to airborne lead (above the action level) would be expected; however, OSHA defines airborne concentrations and lists specific work practices which may create a lead hazard. Therefore, personnel exposure monitoring should be performed during any removal or demolition process to establish worker exposure levels which can be used to determine appropriate personnel protection and environmental controls for the specific work practice and material type. In accordance with OSHA requirements, the Contractor performing the work will be required to conduct this monitoring.
6.2 EPA Regulations
For disposal of construction/demolition debris that have lead, the EPA requires the generator perform testing for lead content of the waste stream to determine proper disposal in accordance with the requirements of 40 CFR 261.11, Criteria for Listing Hazardous Waste.
7.0 PCB Containing Components
7.1 Methodology
During our site visit, H&P was accompanied by SVAMC facility staff who told us that just prior to our survey; they completed a lighting upgrade on the wings of the 4th floor. H&P visually reviewed accessible areas of the wings of the floor and the Central Elevator Lobby area to check for electrical transformers, gear boxes, light ballasts and other components that could contain polychlorinated biphenyls (PCBs).
7.2 Results and Recommendations
Based on our review and the understanding that a lighting upgrade had recently been completed, obvious PCB containing ballasts and other components were not observed. However based on the age of the building, there could be concealed components that could contain PCB’s that may be revealed during demolition for renovation. If encountered, the component should be reviewed and assessed prior to disturbance. If found, PCB’s should be properly disposed following all applicable local, state and federal regulations.
8.0 Mercury Containing Thermostats, Switches and other Components
8.1 Methodology
H&P visually reviewed accessible areas of the Central Lobby, H wing, and J Wing Room 117 to check for suspect mercury containing components including thermostats, switches, fluorescent lamps, mercury vapor lamps, and other components.
8.2 Results and Recommendations
H&P did not observe obvious mercury containing thermostats, switches or other components.
Fluorescent light bulbs were observed throughout the survey area that could contain mercury, including newer green capped bulbs that do contain low levels. H&P recommends that all fluorescent light bulbs be treated as mercury containing and collected for proper disposal following all applicable local, state and federal regulations. Mercury containing waste components are considered as hazardous waste materials in the Commonwealth of Virginia and should be handled accordingly.
9.0 Other Miscellaneous Materials
Medical waste collection containers are present in rooms throughout the project area. These containers should be emptied by qualified personnel and the contents disposed per established facility procedures prior to being disturbed during renovation work.
Other materials of an environmental concern that could require special handling and disposal were not observed in the areas reviewed during this survey.
10.0 Limitations
This report summarizes the results of our sample collection and analysis of the suspect ACM and lead based paint testing and review for other materials of concern based on our understanding of the requested scope of work. The findings prepared by H&P are based upon our observations and the analytical analysis of the samples collected at the time of our field inspection. The services performed were provided in accordance with generally accepted environmental industry standard practices. No warranty, expressed or implied, is made. Our observations, conclusions and recommendations are based upon conditions readily visible at the site at the time of our visit, the results of testing and analysis and information provided to us by others.
Our conclusions and recommendations are based on the guidelines presented by the EPA, Commonwealth of Virginia and OSHA. Any conditions discovered which deviate from the data contained in this report should be presented to us for our evaluation. During H&P’s non-invasive inspection, accessible areas were visually surveyed for the presence of ACM,.
Areas reviewed were limited to those identified by the Client and that could be readily and safely accessed. Our conclusions and recommendations are based on the results of our testing and cannot be used to form a professional opinion of conditions in other areas beyond those from which the samples were collected. It is possible that inaccessible areas, such as behind walls or above ceilings, may not have been surveyed and therefore conditions in these areas are unknown.
During this study, suspect asbestos samples were submitted for analysis at an NVLAP-accredited laboratory via polarized light microscopy. As with any similar survey of this nature, actual conditions exist only at the precise locations from which samples were collected. Certain inferences are based on the results of this sampling and related testing to form a professional opinion of conditions in areas beyond those from which the samples were collected. It is also understood that this is a limited survey so that it is possible that concealed materials may be present that were not accessible to us. No other warranty, expressed or implied, is made.
