LIN ACC VAULTS 1 & 2 MODIFICATIONS
SOLICITATION #36C24619B0021 AMENDMENT #4 7/9/2019
The revisions to the bidding documents are as follows:
1. This procurement is 100% set-aside for Service-Disabled, Veteran-Owned Businesses (SDVOSB). Only SDVOSB concerns are eligible to bid on this project, and bids received from concerns that are not SDVOSB concerns shall be rejected. In order to be considered, the firm must be registered on the U.S. Department of Veteran Affairs VIP website (https://www.vip.vetbiz.va.gov/) as a SDVOSB at the time of submission of their bid. No award will be made to a bidder that is not registered.
2. To allow interested bidders sufficient time to prefer their offers, the deadline for receipt of bids has been extended until Tuesday 7/16/19 at 2:00pm local time. Bid opening will be held in Room 2K-115 located on the 2nd floor of the VA Richmond Main Medical Center (Building 500).
This will be a public bid opening. Hand-delivered bids will be received, opened and read aloud. Bids that are couriered overnight should be sent to the following address:
Mark S. Harville, Contracting Officer
NCO 6
Building 507 05E/90 H.H. McGuire VAMC 1201 Broad Rock Blvd Richmond, VA 23249
PH : (804)675-5841
Bids sent via courier must have documented proof of arrival on station prior to the deadline.
3. Responses to bidder questions and RFI’s received by the deadline Friday, 6/28/19 @ 4:00pm local time are below:
1. Is GPR required in locations that are to receive helical piles?
RESPONSE: Existing underground conditions at pile locations shall be surveyed (GPR) to ensure no conflicts prior to helical pile installation.
2. Will it be possible/allowable to get a mini-excavator or skidsteer inside the building to the locations of the interior helical piles?
RESPONSE: Contractor may consider the use of electric powered equipment for this work.
3. Please verify that the counter top in 1Z-134 is Solid Surface Quartz 1 1/4 (3CM Material). The drawings show this as solid surface 3CM material but the Specification sheet shows all P-Lam material.
RESPONSE: Solid Surface per Schedule for Finishes (09 06 00, 2.6 SS#1)
4. Please provide details/requirements for the approved LINAC equipment to determine pit requirements and electrical locations/details.
RESPONSE: Vendor drawings for Varian and Brain Lab will be provided by the VA. The contractor is required to coordinate pre-installation requirements with each equipment vendor.
5. Sheet AD101 – Note 1 states “Demolish Concrete Slab. See structural drawings for extent of demolition”. Sheet AE112 – Note 10 states “New concrete slab, entire room. See structural drawings”. The structural drawings only show a pit infill in this room. Is the entire floor in this room to be demolished and replaced? Please advise extent of concrete work required.
RESPONSE: The existing slab in the LINAC Vault shall be selectively demolished at the locations of underslab utilities and at the equipment baseframe pit. See attached structural drawings. All locations, dimensions, depths, etc shall be coordinated with the equipment manufacturers and MEP contractors.
6. Is it the intent of the VA to contract directly with BrainLab or are we (the general contractor) to contract BrainLab?
RESPONSE: Equipment will be furnished and installed by Brain Lab under a separate contract with the VA, not under the general contract for construction. The contractor is required to coordinate pre-installation requirements with each equipment vendor.
See attached VA furnished VA installed equipment documentation
7. Is it acceptable to cast lift points into the concrete slabs?
RESPONSE: Lift points may be cast into the concrete slabs but any projections must be cut flush and the lift points infilled solid with concrete.
8. Rather than Precast Structural Pretension Concrete planks (03 41 33) for the roof, is it acceptable to cast concrete planks in place?
RESPONSE: No, precast concrete solid slabs shall be provided.
9. Is it acceptable to core drill the roof to fill the concrete columns?
RESPONSE: No.
10. Per note 5 on SF101, helical piles are to be installed prior to concrete plank installation in order to support the added weight. Demolition Note 16 on AD101 appears to indicate that the pile installation is to take place before Phase 1 is to begin. Will it be acceptable to shut down Vault 1 (Phase 2 work) for an extended period of time during Phase 1 to install the helical piles necessary for Vault 2? What is the intention with how the pile work is to fit into the phasing plan?
RESPONSE: Two LINAC Vaults need to be operational at all times. Vaults 1 & 3 or Vaults 2 & 3. The Contractor is required to phase the work so that this requirement is met. Vault 1 can be shut down to install the helical piles.
11. Which type of roof is the existing roof? PVC, EPDM, etc.?
RESPONSE: Record drawings indicate a Built Up roof is the existing roof.
12. Which type of roof is desired for the new roof work? The specifications call for 60 mil EPDM but the drawings (AE300) call for PVC.
RESPONSE: Please follow the specifications and use EPDM. Installation of transitional components will be needed between existing and new roof.
13. Which type of insulation is desired for the new roof work around, on the side of, and on top of the new planks? The specifications call for R Value 33 and the drawings (AE300) call for tapered insulation.
RESPONSE: R value 33 minimum is required for roof insulation. Tapered to be used to fabricate roof slope on new construction and to match slopes of existing where roofing is replaced.
14. What is the desired warranty for the new roof work?
RESPONSE: See Specification Section 07 53 23, 1.10 Warranty
15. Per Addendum 1 revised drawing AE610, the Finish Notes go 1, 2, 3, 4, 6, 7 skipping 5, however 5 is called out in two separate locations in the finish floor plan in 1Z-134 while there is no number 6 called out on the floor plan. Please advise how we are to interpret.
RESPONSE:
As stated in Amendment #2 issued 6/21/2019, Amendment #1 was rescinded.
Therefore, ALL information on revised drawing AE 610 is to be disregarded.
See attached sketch ASK-1 for corrected new work notes on plan.
16. Will there be an equipment schedule provided with product information and dimensions?
RESPONSE: VA to respond
17. Who are the Brain Lab and Varian equipment vendors? And what is their contact information?
RESPONSE: VA equipment vendors who will furnish and install the equipment.
Equipment will be furnished and installed under a separate contract with the VA.
18. Assuming we are to remove spoils and any equipment out of the hospital through the double door in front of Linac 3, and assuming additional ICRA/containment measures will need to be carried through the hallway to that double door, access to Linac 3 will be inhibited. What is the hospital’s disposition as it pertains to those logistical items?
RESPONSE: The double doors in front of LINAC #3 should not be used. Spoils and equipment should move down corridor C12-4 to corridor DC1Z-2 (east of column line 23) and then out to the existing courtyard. Follow the specifications for ICRA requirements.
19. Demolition Note 1 on AD101 says to see structural drawings for extend of slab demolition. Please reference where on the structural drawings the extend of slab demolition is shown.
20. What is the intent for demolition of the concrete slab in 1Z-132 to allow for helical pier installation? There is no reference to Demolition Note 1 on AD101. Please explain the extent of demolition for room 1Z-132.
RESPONSE: The existing corridor slab on grade (not LINAC Vault slab) shall be selectively demolished as required to allow for helical pile installations as noted on 1/S501. Slab shall be patched/infilled per details on S-501.
21. Regarding 1/SF101, at the back of the rooms the detail shows 4 piles further left than center between the two rooms and only two to the right of center. On the roof plan however, the concrete planks are shown in the center of both rooms. Is the off-center location of the piles able to provide sufficient support for the new concrete on the roof?
RESPONSE: At 25’- 0” spacing (as noted on SF101), 6 piles will be more centered than what was graphically shown on plan. See attached structural drawing. The spacing provides adequate support.
