About this file
This is Amendment 0002-06 to Solicitation 36C10G26R0003 for the CCN Next Gen Medical TOPR (Task Order for Professional Services) West issued by the U.S. Department of Veterans Affairs' OPAL Strategic Acquisition Center in Fredericksburg, Virginia. The amendment, effective March 6, 2026, updates the RFP language, revises Attachment 1 (CCN Next Gen Medical TOPR PWS_West_03_05_26 v3), and publishes a full conformed copy of the solicitation. All other terms and conditions remain unchanged.
The amendment incorporates six substantive changes: (1) modifies Section B.3.1 VA Payment Hierarchy to clarify that when there is no Medicare or VA Fee Schedule rate, the VA reimburses the contractor at 50% of the provider's billed charges "to the general public for similar services"; (2) updates Section E.2 price evaluation criteria to specify that offerors must complete the pricing template in accordance with TOPR instructions and establishes a standardized methodology for converting unit-based and percentage-based pricing into evaluated price values; (3) revises Section E.2 Veteran Involvement/Subcontracting Plan factor to change non-veteran offerors' consideration from "some consideration" to "partial consideration" when using certified SDVOSBs or VOSBs; and (4) through (6) update references to attachments, including updating the Performance Work Statement version from v2 to v3 and moving Attachments 6 and 7 (Disaster Benefit Data and Optional Task Data Historical) from the contract attachments section to the TOPR attachments section.

This amendment provides a conformed copy of the CCN Next Gen – Medical TOPR 0002 West, published with the CCN Next Gen Medical solicitation, noticed 36C10G26R0003. The following revisions have been incorporated into the updated conformed copy and are detailed below for your reference.
Note: The words in red font in the “To” sections of each change denote the new language to be added in each version.
CHANGE 1:
The following language in section B.3.1 VA PAYMENT HIERARCHY is hereby changed as follows:
FROM:
For healthcare services provided, when there is a Medicare rate for the authorized services, the VA reimburses the Contractor the rate up to the maximum allowable, locality-adjusted Medicare rate inclusive of the applicable Medicare assigned deductible. The Contractor shall compare the Medicare rate with the provider’s billed charges and pay the provider. After the Contractor pays the provider, the Contractor shall submit an invoice to VA for payment at the lesser of the Medicare rate or the billed charges. When Medicare applies the CMS Mid-level reduction as determined by the rendering providers taxonomy, the Contractor shall apply that reduction. Reference the
“Attachment 2 - CCN Next Gen Medical Contractor Manual _02_24_26_v2” for additional details regarding the CMS Mid-level reduction.
A waiver of the Medicare rate will apply, as outlined in Schedule of Services below, if the Contractor submits and VA approves a request.
When there is not a Medicare rate for the authorized service(s), the VA Fee Schedule dictates the maximum allowable rate. When there is not a Medicare rate for the authorized services but there is a VA Fee Schedule rate for the authorized services, VA requires the Contractor to compare the VA Fee Schedule rate with the provider’s billed charges and pay the provider and submit an invoice to VA for payment at the lesser of the VA Fee Schedule or billed charges.
When there is not a Medicare rate or VA Fee Schedule rate for the authorized service(s), the VA shall reimburse the Contractor at 50% of provider’s billed charges.

CHANGE 2:
The following language in section E.2 ADDENDUM to FAR 52.212-2 EVALUATION— COMMERICAL ITEMS, Factor 4: Price, Evaluation Criteria, is hereby changed as follows:
FROM:
For this factor, the Offeror’s price proposal will be evaluated based upon the proposed unit prices and the Government’s calculated Total Evaluated Price (TEP). All proposed unit prices and the TEP will be evaluated for completeness and to determine whether the proposed fixed prices are fair and reasonable in accordance with FAR 15.404-1(b).
The TEP will include the sum of prices for the base year and all option years. Evaluation of options shall not obligate the Government to exercise such options. Price will not be evaluated adjectivally or assigned a score.
(a) Completeness: The Government will review the pricing submissions for completeness. A proposal is complete if the Offeror used the Government-developed Price Schedule Excel file as required, provides Cost/Price data for all solicitation requirements as required by Section E, the proposed costs are traceable and accurate, and the pricing table does not include blanks, unreadable files, or incomplete data. The proposed costs are traceable if they can be tracked from the Basis of Estimates to the Price Schedule, as well as throughout the Price Schedule. Incomplete price submissions may not be evaluated, and the proposal may be eliminated from the competition.
Government will NOT compare offers for the first year against offers for the entire multiyear period.
(b) Reasonableness: The Government will perform analysis using one or more of the price analysis techniques identified in FAR 15.404-1 to determine reasonableness.
(c) Total Evaluated Price (TEP): For the purpose of determining Best Value, the
Government will use the TEP for each Offeror. The TEP will be the sum of all priced CLINs.
(d) Options: As part of price evaluation, the Government will evaluate its option to extend services, FAR Clause 52.217-8, by adding one-half of the Offeror's last option period price to the Offeror's total price. The Government will evaluate proposals for award purposes by adding the total price for all performance periods. Evaluation of the option shall not obligate the Government to exercise the option. The Government may determine that a proposal is unacceptable if the option prices are significantly unbalanced.
(e) Unbalanced Pricing. The Government may evaluate whether the Offeror has submitted unbalanced pricing. An unbalanced price is one where the price of one or more contract line items or components is significantly overstated or understated, and which will result in the Government paying an unreasonably high price for contract performance or otherwise present an unacceptable level of risk to the Government. If the Contracting Officer determines that the risk is unacceptable, then the proposal may be rejected.
No price realism analysis will be conducted.
TO:
For this factor, the Offeror’s price proposal will be evaluated based upon the proposed unit prices and the Government’s calculated Total Evaluated Price (TEP). Offerors shall complete the pricing template in accordance with the TOPR instructions.

(b) Reasonableness: The Government will perform analysis using one or more of the price analysis techniques identified in FAR 15.404-1 to determine reasonableness.
(c) Total Evaluated Price (TEP): To determine Best Value, the Government will use the TEP for each Offeror. For purposes of evaluation, the Government will calculate the Total Evaluated Price (TEP) using a standardized methodology that converts all proposed pricing inputs including unit-based pricing and percentage-based pricing into evaluated price values. Where percentages are required, the
Government will apply the Offeror’s proposed percentage rates to the applicable pricing assumptions, estimated quantities, or evaluation scenarios defined within the pricing template to derive a comparable evaluated dollar value. The TEP will then be calculated as the sum of all priced CLINs using this standardized approach.
If the Contracting Officer determines that the risk is unacceptable, then the proposal may be rejected.
No price realism analysis will be conducted.
