About this file
This is a Medical Waiver of Payment Rates Request form used by the U.S. Department of Veterans Affairs (VA), Veterans Health Administration, Office of Integrated Veteran Care (IVC) to evaluate contractor requests for payment rate increases due to network adequacy concerns.
The form requires contractors to complete Parts I through III with submission details including the contractor purchase order number, affected CPT/HCPS codes, VISN, VAMC, current and proposed rates, and scarcity/rurality designation. Part II requires comprehensive justification narratives that must include: NPI(s) and Tax ID(s), specific provider location addresses, number of providers in the practice, number of Veterans served in the last six months, specific codes requested for waiver, and detailed justification explaining the number of currently contracted providers in the catchment area, services offered, and potential impact to Veterans if the provider leaves the network. Part III requires a Table of Non-Contracted Providers listing all outreach attempts to other providers within or near the VAMC catchment area, including provider details (name, NPI, street address, city, state, ZIP code, phone number) and the reason for non-contracting, selected from predefined options such as provider no longer practicing, capacity constraints, sanctioning status, or inability to contract. The form notes that "Inability to contract with provider" is not a valid sole rationale for the request. Part IV, completed by VA internal review, documents recommendations from the Rate Waiver Review Board and Network Adequacy team, along with the Contracting Officer's decision, decision date, and expiration date if applicable. The form was revised in January 2026.
