VA Minneapolis Cardiac PAC’s SOW
Summary:
VA Minneapolis seeks a software application designed to help streamline clinical workflow and improve operational performance across your entire cardiovascular service line. This is to be a turnkey hardware and software solution.
The solution must provide instant access to all relevant cardiovascular images and information anytime, anywhere within the VA Minneapolis. To help improve patient cardiac care and work flow, a graphical, panoramic, chronological overview of a patient’s cardiovascular care continuum should be able to be displayed. This should be provided to the user in a patient-centric environment that allows intuitive navigation through images, reports, trends, and analysis. This environment should include views of current and previous studies and all cardiac imaging modalities including cardiac catheterizations, vascular ultrasounds, echocardiograms, cardiac CT studies, cardiac nuclear medicine studies, electrocardiograms, cardiac ultrasounds, and intravenous ultrasounds.
The system must supply access to advanced clinical tools to perform comprehensive analysis and reporting to allow for faster and more effective cardiac care treatment decisions.
The system must include utilities for system administration that allows customization of the workspace, configuration tools, lists, and user access and privileges.
The system must perform all necessary functions for exam storage and review for echocardiography, cardiac EP and interventional catheterization labs including interfaces to associated systems such a hemodynamics, EP mapping, and 3-D modeling.
The system must be robust, fail safe, and easily scalable as facility needs change and evolve over time.
Exam workflow:
The system must support standard DICOM functionality, including: importing exams from DICOM image acquisition devices, DICOM Store, Query/Retrieve and DICOM Forward.
DICOM exams can be stored and displayed as defined in the DICOM Conformance Statement.
System prompts, menus, and notifications must be intuitive for users to experience a smooth, undaunting transition to the new system, without excessive training or technical support.
The system must employ VISTA and VISTA imaging certified interfacing and worklist management for exam workflow, ADT, and orders interface
The system must be able to support cardiovascular X-ray, ultrasound, intra-venous ultrasound, nuclear medicine, computed tomography, magnetic resonance, 3-D printing, hybrid OR, and electrophysiology examination types, peripheral intervention, vascular analysis software, Cardiac CT myoperfusion, nuclear (SPECT) perfusion.
Synchronized display of study images with ECG.
The system can also export exams to other systems using industry standard DICOM Store protocols to encourage, advance, and promote collaboration amongst medical center Cardiology departments. The current PACs systems utilized by the Minneapolis VAHCS are third party, vendor-neutral ACOU/VNA.
The system must facilitate and streamline a process for facilities within VISN 23 to collaborate and refer workload between each other.
Reporting & Analysis:
User customizable report forms and templates. Robustly intuitive user interface for ease of learning and use.
Utilizing configurable decision support and “smart” report auto filling utilizing cardiology industry standard parameters in anatomy measurements and motion, flow, and calculations. These parameters must be configurable as normal/abnormal ranges can change over time and site.
Users must be able to create user-defined measurements and calculations as well as edit labels for the default set of measurements and calculations.
Measurements and parameters must pre-populate a choice of pre-configured clinical report templates that include report formats.
Sets of clinical statements and measurements must be able to be entered with an abbreviated “short hand” method to reduce entry and typing time as well as standardized report language. The standard clinical statements and measurement must be a customizable for site-specific protocols and requirements.
Comprehensive set of data mining tools with both preprogrammed and easily customizable search functions.
The system must support user defined equations and analysis of procedures.
Capability for 3-D modeling and printing of patient anatomy.
Technical:
The system must be able to store and retrieve a minimum of 7 years of estimated facility workload of exam images, video, and reports on disk based technology. The current Minneapolis VAHCS Cardiology PACs storage system is utilizing 29TB of data and is sized for 60TB total.
The solution must include all hardware and software required to complete the system. The current inventory of the Minneapolis VAHCS are 30 workstations and monitors. Proposed server, workstation, and monitor technical specifications to be submitted for comparison purposes.
Fully compatible to accept echo and doppler information from all major manufacturer echocardiography devices. Fully compatible to accept imaging studies from all major manufacturers imaging modalities including x-ray, angiographic, magnetic resonance, CT, nuclear medicine, PET, as well as hybrid combinations of these modalities.
System must utilize a client/server cardiology workflow solution integrating the imaging modality, VISTA EMR, viewing, reporting and analysis workstations, together with the server base software solution.
Robust fault tolerant server system design enabling fail-over and self healing to help assure minimum down time and continuous use.
Web link application access so that workstation footprint is minimal.
Fully HL7 (inbound and outbound) and DICOM compliant
Proven integration with VA VISTA and CPRS.
Fully operational with the most recent VA approve server and workstation operating systems. Minimum acceptable OS must be OEM supported.
Use of fault tolerant redundant storage technology to assure system integrity, longevity, and reliability.
Must a have the ability to migrate studies from existing legacy Siemens system. Estimation of existing Siemens system is 29TB of data to be migrated of both echocardiography and cardiac catheterizations on to spinning or solid state disk arrays.
Full and robust warranty and service/support options. This must include proven VA remote access support capability.
The proposal shall include a thorough response on how the proposed system addresses the image evaluation/manipulation tools listed in the chart below. The toolset chart is not necessarily all inclusive.
If the proposed system has additional cardiology tools, please provide a description in response as well.
Training:
Comprehensive clinical training plan and offerings. Role base offerings, minimally super user and regular user. Others as system design dictates.
Hands on participative training so that users can experience operation first hand from the start.
Comprehensive technical support training for a minimum of 2 individuals.
On site “go-live” training and support (clinical and technical)
Offering both on-site, hands on and video or web based user training.
PACs administrator training included.
Value added:
Software is available to retrieve electrocardiograms from MUSE ECG management system.
Additionally offers various analysis and quantification packages, clinical reporting and archiving features as optional functionality.
Employs open architecture and industry standards-based design and is prepared for future growth. It will provide ready access and availability to past and present exams as well as final reports (if configured) as needed
Flexible software licensing structure for ease of multiuser access, maintenance, and expansion.
PIV compatible login: (one of the following)
a. direct PIV certificate based authentication
b. Integration with IAM (VA Identity and Access Management) services to perform Single Sign on (SSOi) authentication.
c. Active directory integrated windows authentication (AD-IWA)
DICOM archive connectivity, which provid.es the ability to automatically store image studies on an external DICOM archive, such as a radiology PACS system with minimal loss of study features, effects and enhancements.
Support for patient-procedure fusion imaging ( CT-fluoro, echo-fluoro, etc.)
Software has capability to integrate with third-party voice recognition software. The Minneapolis VAHCS utilizes Nuance Dragon voice recognition dictation software.
Integration with VA CARTCL application.
Cardiology PACs solution has the ability for remote access for telework capabilities for providers.
Extended software maintenance agreement.
