About this file

This document is an Attachment C - Proposer Information and Certification Sheet for RFP 34366 - Weigh-In-Motion Equipment issued by the State of Oregon. The RFP is seeking proposals from vendors to provide weigh-in-motion equipment. Proposers must submit a completed Proposer Information and Certification Sheet that includes contact information, legal certifications, and references for comparable projects. The references will be evaluated based on ten criteria such as responsiveness, reliability of components, budget management, and schedule performance. Proposers must also disclose any potential conflicts of interest.

The RFP is part of a state and local contract opportunity in Oregon for weigh-in-motion equipment. The contract will likely be awarded to a single vendor, though the RFP does not specify the contract value, term length, or renewal options. Funding requirements and sources are also not provided in this document. The RFP does indicate that the State may not enter into a contract over $150,000 with a proposer that does not have a written policy to prevent sexual harassment, sexual assault, and discrimination.
