About this file

This document is a Visual Inspection Report and a set of related documents for a federal construction contract opportunity.

The Visual Inspection Report details the findings from an inspection of structural steel components for a construction project, including issues with welding procedures, quality of workmanship, material conditions, and required repairs. It notes concerns with the welding process, assembly techniques, and material preparation. The companion Smithsonian Project document provides additional guidance on welding requirements, positions, and procedures to be followed.

The related federal contract opportunity is for the Construction of a Maintenance Shop located in Gamboa, Republic of Panama, with an estimated construction budget up to $1.2M. The Solicitation Number is 333302-24RF-0010029, the NAICS code is 236220 Commercial and Institutional Building Construction, and the contract will be federally funded. The Smithsonian Institution is seeking technical and cost proposals, which are due by July 12, 2024 at 3:00PM EST. The successful offeror must be legally registered and licensed to operate as a construction company in Panama for at least 5 years and have performed at least three construction projects valued at $1M or more in the past 3 years.

Observations

• Upon arrival to the contractor shops in calzada larga a WPS – welding procedure specification as per AWS D1.1.

2020 / steel was requested, the document presented by the shop representative does not apply to a full penetration joint as mandated by the project design.

• During the visual inspection the following was noted: welds with irregular configuration, welds spatter patterns, undercutting on the welds side and back of experience in finishing the weldments.

• Fillet welds were accepted because they met the minimum acceptable criteria with the exception of one having a low profile, it was marked and identified for repair and re-inspection

• All butt welds between beams splices were found acceptable according to the acceptance criteria in the welding code

• All marked repairs were corrected during our inspection visit, re inspected and accepted except two with a pending status.

• The dye penetrant testing procedure indicated slight undercutting patterns on the border of the weldments and a tendency for lack of fusion. The discontinuities were marked for repair using a grinder to remove the condition and a re-weld.

• During the UT testing and evaluation to all Butt joints between beams no major flaws or defects were found. All four butt joints were accepted.

• No UT test evaluation were applied to the plate – column connection because it was not a complete joint penetration as mandated by the project specification.

• Other observations:

o All cuts with the oxy – gas process showed poor appearance and lack of experience in the flame cutting process, poor quality equipment, cutting torches, rough edges and limited supervision over the shop production.

o Corrective grinding is required in most oxy – fuel cuts to plates for column.

o We observe poor craftmanship in the assembling of structural elements with clear examples of out of squareness, out of plombs, warpage, distortion, hi-low and tendency for misalignment.

o The steel material showed unacceptable surface conditions from the delivery to the contractor shops, this is typical of material left unprotected and stored in the open.

• As per information sent via drawings approved for the Smithsonian project:

1. The welding symbol for the fillet welds is presented incorrectly drawn, must comply with AWS A 2.4 standards.

2. All fillet welds as drawn in the submitted prints must be carried out as final connections so as to case erection procedure to avoid out of squareness, out of plomb and misalignments, especially those fillets with 1 /2" and 3/8" effective throats. During our field inspection visit to the contractor shops located in calzada larga on June 21 of 2022 we noticed serious inconsistencies, lack of experience in assembling techniques for structural steel components, lack of rigs and special benches, most works were put together using the floor as a mold visible out of squareness, distortion due to poor welding techniques and in some cases poor and limited craftmanship.

3. As per the drawing it seems very difficult to provide an effective throat of 3/8” size on the border of the joint.

4. Filler metal should be E – 7018 H4R to assure structural quality as per AWS D1.1 –

2020 / steel. In order to obtain an 3/8” throat, several oases with 1/8” diameter electrode is recommended, heat input is a major consideration.

5. The welding connections between the 8” beam and the 12” beam are to be welded in the 3F position, vertical up progression.

6. The 4" X 4"X 3/16” square tube must be welded in the 3 F – vertical up progression, in the 4 F – out of position welding or overhead and in the 2F position – horizontal.

7. The 4 anchors bolts B7 implies the welding of the column plates with a combined plug weld and a final fillet weld, this procedure must be submitted for revision and final approval on behalf of the owner or its technical representative. It’s anticipated the usage of shims to attain levelness. special grouting would be required.

8. A special anchoring technique is recommended so as to control speed in drilling and the avoidance of fracture.

9. Before the connection between the 10” beam and the 12” beam through the stiffener plate verification is required to make sure that the 5mm´s fillet welds are feasible and that all drainage designated locations are made so as to avoid corrosion and trapper’s water.

10. Verification of all welder’s certification is required for the job. They will be welding grooves and fillets joint in the:

1 G 1F flat

2 G 2F horizontal

3 G 3F vertical

4 G 4F overhead

11. Surface preparation and painting is expected, except where a final connection by welding is to be done.

12. All welds between galvanized iron C – channels in the roof section must be done free of holes, burns or excessive heat, E-6011 – 3/32” diameter of F3 Group is recommended with low amperage thus controlling heat input. All weldments must be cleaned and paint protected as soon as the heat in the heat affected zone allows. It is best to paint protect the same day the welding was done.

• As per the ultrasound evaluations:

1. According to the report sent by Ademinsa signed by their representative the UT equipment used to preform the test has a valid calibration certification for one year from February 2022 thru February 2023.

2. According to the TECMEC S.A. UT report all tests and evaluations were accepted except one where a lack of fusion between passes was located, it was marked and identified. Was repaired and UT re-inspected and accepted.
