About this file
This is a Cost Proposal Form (Attachment 3) for Salt Lake City Corporation's Request for Proposals #SLCI27031 for Micromobility Tracking Software. The form requires offerors to submit comprehensive pricing for a cloud-based dashboard software application designed to track dockless shared mobility devices within Salt Lake City's municipal jurisdiction. The City seeks a solution to monitor, audit, regulate, and report on dockless mobility device usage to support program goals including reducing automobile vehicle miles traveled, promoting safety and Vision Zero alignment, and advancing transportation justice. The contract term is five years, and all costs necessary to implement and maintain the proposed system over this initial contract period must be included in the cost proposal.
The Cost Proposal Form requires offerors to detail pricing across five cost categories: initial and implementation setup, licensing, annual maintenance, initial user training, and other applicable costs for each year of the contract term (Years 1–5). Additional sections require hourly rates for configuration, training, developer/customization services, and optional future enhancements. All cost entries must be reflected in the Technical Requirements proposal, and any costs not provided on the Cost Schedule will not be allowed into the resulting contract. The form specifies that the Total Proposed Cost (highlighted in green) combines implementation costs and annual maintenance expenses and will be evaluated as the primary cost criterion. Offerors are instructed that zero entries or blank cells will be interpreted as services provided at no cost or built into another line item, and any deviation from the prescribed format may result in proposal disqualification.
