About this file
This appendix document from the City Colleges of Chicago provides the IRS Form W-9 (Request for Taxpayer Identification Number and Certification) along with comprehensive compliance requirements for the Washburne Culinary & Hospitality Institute's Point of Sale System procurement in Illinois. The document outlines the required documentation and certifications that bidders must submit, including the standard W-9 form used to collect taxpayer identification information from vendors and contractors. The appendix integrates detailed policies regarding Minority Business Enterprise (MBE) and Women Business Enterprise (WBE) participation goals, with bidders required to complete multiple schedules (Schedule A through Schedule D) documenting their commitment to subcontracting with certified MBE and WBE firms, including letters of intent, certification determinations, and joint venture agreements if applicable.
The compliance framework establishes mandatory participation goals for MBE and WBE firms, requiring bidders to demonstrate commercially useful functions for all subcontractors and maintain detailed record-keeping throughout contract performance. All vendors and contractors must complete ethics training and sign an acknowledgment of the City Colleges of Chicago Ethics Policy, addressing fiduciary responsibility, use of district property, and political activity restrictions. Additionally, bidders must complete an Economic Disclosure Statement and Affidavit as part of the proposal submission requirements. The document emphasizes that failure to meet MBE and WBE participation goals may result in non-compliance determinations, with available remedies and sanctions enforceable by City Colleges of Chicago, including the right of contractors to appeal adverse decisions.
