About this file

This is a Contractor Performance Assessment Reporting System (CPARS) form template required by the Department of the Army (DoWEA) Procurement Transportation Division as part of federal contract representations and certifications.

The form requires contractors to designate a representative who will receive and review Government performance assessments electronically via www.cpars.gov. The designated representative must provide their name, title, email address, telephone number, and N/CAGE number for authorization access. This representative has authority to receive performance assessments from the Assessing Official, review and comment on assessments, and return them within 30 days. If the contractor requests a meeting to discuss the evaluation, it must be submitted in writing no later than seven calendar days from receipt. The form specifies hardware and software requirements for CPARS Online access, including support for the latest three major desktop versions of Internet Explorer and the latest versions of Chrome, Firefox, and Safari, along with Adobe Acrobat Reader installation. Technical support is available through the Customer Support Desk at voice phone (207) 438-1690 or DSN 684-1690, with additional support available at webptsmh@navy.mil. The contractor is solely responsible for notifying the Assessing Official or Contracting Officer of any changes in designated personnel.

Hardware/Software Requirements for CPARS

To use CPARS Online, you will need to have access to the internet along with the following hardware and software:

CPARS fully supports the latest three major desktop versions of Internet Explorer, and the latest major desktop version of Chrome, Firefox, and Safari. Older browsers may still view CPARS, however users should expect mixed results. A "major version" refers to a full numeric release, like 9.0 and 10.0 (not minor releases like 9.2.x and 10.2.x).

The Adobe Acrobat Reader should be installed. This allows you to view and print the PDF attachments in

CPARS and FAPIIS. The reader can be downloaded and installed from Adobe Acrobat. If you have any problems downloading or installing the reader, please contact our Technical Support.
