About this file

This is a response template attachment for RFP #22025 Media Services issued by a state lottery agency. The document serves as a standardized form for proposers to detail their organizational structure and staffing qualifications in response to the media services solicitation. Proposers are required to identify Key Persons for four specified roles: Account Manager, Media Director, Media Planner/Buyer, and Media Supervisor/Analyst, with the ability to assign multiple individuals to single roles or consolidate multiple roles under one person as needed to meet the lottery's minimum and desired qualifications. The template mandates detailed staff experience documentation for each proposed Key Person, including years of relevant experience, project scope overviews, and professional resumes limited to one page per individual.

The document further requires proposers to describe the geographic locations where work will be performed and to justify their proposed approach to collaboration with the lottery, including specific tools and methodologies for effective remote or hybrid coordination. Proposers must also document assumptions related to their response in a structured table format, referencing specific RFP sections to support their proposed approach. The template explicitly warns that changes to any pre-completed cells will result in proposal disqualification, emphasizing strict compliance with the prescribed format and instructions.
