About this file
This is an E-Verify Affidavit form required by the Board of Public Education for the City of Savannah and the County of Chatham (Savannah-Chatham County Public School System) for the Data Cabling for Hubert Middle School project. The affidavit serves as a contractor compliance document under Georgia state law O.C.G.A. 13-10-91(b), whereby contractors executing the form certify their registration with and authorization to use the federal E-Verify work authorization program. Contractors must provide their federal work authorization user identification number and date of authorization on the form. The affidavit requires contractors to commit to continuous use of E-Verify throughout the contract period and to ensure that any subcontractors performing physical services similarly provide compliant affidavits. The form must be executed under penalty of perjury and notarized, with spaces for the bidder's legal name, authorized representative signature, printed name, and title or position.
The affidavit is a mandatory submission requirement for the low-voltage infrastructure services project at Hubert Middle School with a one-year initial contract term and two optional one-year renewal periods. This document establishes a compliance baseline ensuring that all contractors and subcontractors working on the project maintain active E-Verify authorization in accordance with Georgia state statutory requirements. The notarization requirement and penalty of perjury language underscore the legal significance of the attestation, making it a critical component of the proposal package that contractors must complete accurately and submit with their sealed bids by the August 25, 2026 deadline.
