About this file

This is a Capability Form (Appendix A) template that the Federal Aviation Administration requires interested vendors to complete for the UPS III and Life Cycle Support procurement. The form is designed to assess vendor qualifications and past performance relevant to delivering uninterruptible power systems and associated services.

Vendors must provide detailed information on three previous contracts (A1, A2, A3) and three current contracts (B1, B2, B3) similar in size, scope, and complexity to the draft Statement of Work, including program titles, customer names, points of contact, project dates, contract values, and descriptions of work performed. The form requires vendors to confirm their ability to provide all required services and equipment across multiple categories: personnel, program management, hardware, software, logistics, and training. Additionally, vendors must demonstrate specific UPS capabilities by indicating which contracts involved static UPS units (40kVA-160kVA, 160kVA-225kVA, and 225kVA-550kVA), maintenance bypass panels, valve-regulated lead acid (VRLA) batteries, vented lead acid (VLA) batteries, and lithium-ion batteries. Vendors must provide manufacturing lead times for each equipment category and mitigation strategies for any lead times exceeding 12 months, along with communication plans for delays. The form also requires information on subcontractor involvement, geographic service capabilities across nine FAA regions (Alaska, Central, Eastern, Great Lakes, New England, Northwest Mountain, Southern, Southwest, and Western-Pacific), and on-site technical support availability in both continental and overseas locations. Finally, vendors must provide feedback on Appendix B (technical specifications), their approach to equipment refresh and obsolescence management, availability of lithium-ion battery options with comparative advantages or limitations, and any alternative solutions or concerns regarding the draft Statement of Work.

In order to make the best determination, the FAA requires interested vendors to fill out Appendix A addressing the following and per the instructions below:

· Capability Questions start in Block A.

· Interested vendors may reference the same contract (A1, A2, A3, B1, B2, B3) for multiple citations.

· Supporting document attachments are permitted, provided they reference the specific contract (A1, A2, A3, B1, B2, B3) citation.

· If work was performed on a specific Task Order, cite both the Task Order number and the Base/Parent Contract number.

This form can be completed on government and/or commercial sector projects. Scope may be represented on one contract/project or be represented on separate contract(s)/ project(s) providing same or similar services; within ten (10) years of the issuance of the Market Survey.
