About this file

This is a Proposer's General Questionnaire for UT Health San Antonio (UTHSA), a Texas-based academic health center seeking qualified third-party vendors to provide bad debt collection services. The solicitation (RFP 745-26-P13) targets collection activities for self-pay patient accounts across UTHSA Physicians, the Multi-Specialty & Research Hospital, the Medical Management Program, and the School of Dentistry. The questionnaire requests comprehensive information from proposers including company profile details, financial stability documentation, customer references from similar service engagements within the past five years, and a detailed work plan with key milestones and implementation timelines. Proposers must also disclose any litigation history, current financial defaults, business transactions, and relationships with UTHSA employees. The document specifies that proposers must address the scope of work requirements outlined in Section 5.4 of the RFP and provide summary resumes for key personnel assigned to the project.

The questionnaire requires proposers to address service approach methodology, quality assurance programs, service support philosophy, and disaster recovery or contingency planning capabilities. Proposers must declare their position on providing cloud computing services as part of their proposal response and identify any additional services or unique benefits they would offer beyond the solicitation requirements. The document mandates disclosure of ownership information for any individuals holding at least 25 percent ownership interest in the proposing company for child support delinquency verification purposes under Texas Family Code Section 231.006. UTHSA explicitly states that no guarantee of volume, minimum, or maximum compensation is provided under this solicitation. All responses must be clearly presented in the formal proposal submission rather than in the questionnaire template itself, except for the cloud computing services declaration which must be completed directly in the document.
