Amendment No. 5 Attach – Q-A-3
15-102-SOL-00009
1. Q - Section 8.4 of the geotechnical report states that onsite soils may or may not meet Select Fill and Engineered Fill requirements. Please confirm that on-site soils are suitable for select and engineered fill or confirm that the contractor would be monetarily compensated for the additional costs of soils export and soils import.
The pre-award cost could result in a substantial savings or cost to the Government for this ambiguity if it is not clarified prior to the bid due date.
A - Bidders are hereby advised to formulate their bids with the assumption that 5,000 CY (Cubic Yards) of the native material will be unsuitable for engineered and/or select fill and will therefore require import of suitable soil from an approved source and export of the excess native soil.
2. Q - Bid package includes Prevailing Wage document but there is no class for fire alarm installer or low voltage installer. Typically one of these are listed. Please confirm these class employees are not part of the Wage Classification and that we are to use the electrician rate.
A - – If a classification of laborer is not listed on the wage determination it is at the bidder’s discretion and risk to determine the wage rate to be utilized in developing the bid price.
3. Q - If available, please provide Volume B1 – Medical Equipment and Furniture Manual, Room by Room Listing and Volume B2 – Medical Equipment and Furniture Project Manual, Data Sheets in Excel format.
A - Room_by_Room_Equipment_List provided in Excel Format as part of this Amendment No. 5.
4. Q- a. A potential bidder on millwork is not an AWI certified shop. Per Spec Section 06 40 23
1.5 they are excluded from bidding this project. Is there any variance from this requirement?
b. Sheet E301B, Note 7, calls to refer to radiation equipment manufacturer’s instructions for more information. We are unable to locate these documents. Please clarify.
c. Sheet E301B, Note 28, calls to refer to Shimadzu site specific installation instructions for more information. We are unable to locate these documents. Please clarify.
d. Hemco Corporation, Hood, Fume, Canopy; CAD ID: C26774 – The spec sheet provided indicates both an island mount and a wall mount. Please clarify which mount is required.
A - a. Bidders are instructed to follow the requirements of the design package for bidding purposes. Variance requests will not be entertained during bidding.
b. and c. Regarding the Radiation Equipment (Shimadzu X-Ray Unit), the information is found in Attachment D2: Volume B2 – Equipment and Furniture Data Sheets, on pp 1168-1181.
d. It’s a wall-mounted canopy hood, as highlighted on the Data Sheet.
5. Q- Per Addendum 4, the geotechnical report stamped June 26, 2015 states that it is an update to an initial report dated April 2008. The June 26, 2015 report includes only three (3) borings. We believe that the April 2008 report most likely included many more borings and also remolded swell tests (ASTM D4546) of the existing soils. There is no indication that those tests were conducted on the existing soils that were analyzed in the June 26, 2015 report. Will you please provide a copy of the April 2008 report? That report will assist us in better determining the types and properties of the existing site soils.
A - Bidders can download Geotech Report_April_2008, included with Amendment No. 5.
