About this file

This is a Functional Assessment Plan (FAP) for finishing of hardwood floors (specification item 1503010), establishing performance standards and assessment procedures for contractor work on gymnasium floors.

The FAP outlines detailed performance objectives and standards across multiple service categories: scheduled services ensuring clean floors with uniform shine completed per contractor schedule; floor care requiring sanding of all finish layers to raw wood, dust containment, application of penetrating sealer and two finish coats, reapplication of court markings and NCAA volleyball lines with feathered edges, and installation of new vented cove base; dust mopping to remove debris and dust; buffing to prepare surfaces for refinishing; touch-up painting of court lines, lettering, and logos; and finishing floors with minimum two coats of finish applied after touch-up work is complete. Non-recurring work may be ordered via WAFA or task order with specified locations, work types, and performance periods. Assessment uses three levels with escalating oversight: AL1 (initial level) employs periodic sampling, validated customer complaints, and unscheduled visits with 100 percent sample size on an as-required frequency; AL2 is added if AL1 performance is unsatisfactory; AL3 is added if AL1 or AL2 performance is unsatisfactory. The monthly performance assessment summary template requires rating each specification item using categories: Excellent, Very Good, Satisfactory, Marginal, or Unsatisfactory, with documentation of samples examined, validated customer complaints, and safety issues, concluding with overall technical ratings and recommended actions signed by the Service Area Project Representative.
