About this file
This document provides procedures for updating a vendor file for federal contracts. It includes forms for updating vendor information such as the FMS Vendor File Update Request Form, VA Form 10091, and IRS Form W-9. These forms collect details including vendor name, address, phone number, tax identification number, point of contact, payment information, and small business designations. The document outlines requirements for submitting these forms as part of a proposal or offer for federal contracts unless the vendor has previously submitted the information and provides certification of past work. It emphasizes including the FMS form could preclude consideration for award or delay the award process. Vendors must also be registered with the Central Contractor Registration and checked against exclusion lists.
The related federal contract opportunity is a solicitation for construction services to renovate lab spaces at the Tibor Rubin VA Medical Center in Long Beach, CA. The contractor will utilize provided construction documents and specifications to alter real property. The solicitation and attachments provide full details on the project requirements and response. The Department of Veterans Affairs Veterans Health Administration VISN 22 is the contracting agency.

VHA STANDARDIZED VENDOR FILE PROCEDURE
"Prospective Offerors are notified that verification of VA Form 10091 and the IRS Form W-9 sent directly to the paying office in Austin, TX. must be submitted with the offer unless the offeror has previously submitted these forms to VA Austin and provides certification of previous work. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award.
