About this file
This file is an Affidavit of Compliance template issued by the Town of Palm Beach Procurement and Contracts Division, applicable to the Mandel Recreation Center Fitness Room Flooring Project in Palm Beach, Florida. The affidavit serves as a mandatory compliance document for contractors bidding on the fitness room flooring construction project, which involves removing existing flooring, preparing the subfloor/surface, and installing new rubber flooring. The work is scheduled to commence and be completed between August 1-10, with a non-mandatory pre-bid meeting and site visit scheduled for May 26, 2026, at 2:00 PM EST at the Mandel Recreation Center located at 340 Seaview Ave, Palm Beach, FL 33480. Online bid submissions are required through the Euna Procurement (Bonfire) platform, and the award will be made to the lowest responsive and responsible bidder meeting all stated requirements. Quoted prices must remain valid for ninety (90) days from bid closure.
The affidavit requires bidders to attest under penalty of perjury to compliance with Florida foreign entity laws and anti-human trafficking statutes. Specifically, the undersigned must certify that the entity is not owned or controlled by a foreign country of concern as defined in Florida Statutes Sections 287.138 and 692.201; is not organized under foreign laws; is not a foreign principal; and complies with all applicable requirements regarding foreign ownership restrictions. Additionally, the affiant must confirm that the entity does not use coercion for labor or services as defined in Section 787.06, Florida Statutes concerning human trafficking. The affidavit must be executed by an authorized representative of the bidding entity and notarized by a Florida notary public, with execution occurring before or online. This compliance document is a prerequisite for bid consideration and reflects the Town's adherence to Florida statutory requirements for vendor eligibility.
