About this file
This is a Solicitation Evaluation Criteria document (SPB Form 31) for solicitation number 120084 O5 regarding STD and LTD Insurance services, with an opening date of Thursday, December 19, 2024. The document outlines four mandatory requirements that bidders must meet to be considered for evaluation: a signed Contractual Agreement Form (manual ink or DocuSign), Corporate Overview, Technical Approach, and Cost Sheet.
The evaluation scoring system allocates a total of 100 possible points across three main categories: Corporate Overview (25 points), Technical Approach (65 points), and Cost Sheet (10 points). An additional 20 points may be awarded through oral interviews if required, bringing the potential total to 120 points. The Cost Sheet points are calculated using a formula where the lowest cost submitted receives maximum points, and other submissions are scored proportionally using the formula: Lowest Cost Submitted ÷ Cost Submitted × Maximum Possible Cost Points = Cost Points to Award. The document includes three sample calculations demonstrating this formula with different cost scenarios.
