About this file
This document provides procedures for standardizing the vendor file for Department of Veterans Affairs contracts. It requires prospective offerors to submit an IRS Form W-9 and VA Form 10091 directly to the VA payment office in Austin, Texas with their offer, unless they have previously submitted these forms and can certify past work. Offerors must also submit an FMS Vendor File Update form to the contracting officer with their proposal. This form collects vendor information including tax identification number, DUNS number, contact information, existing contracts if any, business type, socioeconomic classification, and registration with the Central Contractor Registration and Excluded Parties List System. Failure to provide the required forms could delay or preclude award.
The related federal contract opportunity is a solicitation for construction services to prepare the site and install required utilities for a pharmacy sterile compounding trailer at Building 164 of the VA Long Beach Healthcare System. The requirement is classified as construction and has a 100% set-aside for service-disabled veteran-owned small businesses. The solicitation number is 36C25820B0014 and interested vendors should review all attached documents. The Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 22 is the contracting agency.

VHA STANDARDIZED VENDOR FILE PROCEDURE
"Prospective Offerors are notified that verification of VA Form 10091 and the IRS Form W-9 sent directly to the paying office in Austin, TX. must be submitted with the offer unless the offeror has previously submitted these forms to VA Austin and provides certification of previous work. The FMS Vendor File Update form needs to be submitted in all cases to the Contracting Officer as part of the offer or proposal. Prior information sent to the VA Payment Center in Austin does not preclude submittal of the FMS Vendor File Update form. Failure to include this information could preclude the offeror from consideration for award or considerably delay award.
