About this file

This is a Location Certification form for Broward County, Florida, used to establish vendor eligibility for local preference and tiebreaker advantages in competitive solicitations. The form applies to this Architectural and Engineering Consultant Services solicitation, which seeks three professional architectural firms licensed under Florida Statutes Chapter 481 to provide architectural and engineering services on a continuing basis for vertical construction projects including remodeling, renovation, and new construction. Vendors must submit the completed Location Certification form and all required supporting documentation by the solicitation end date, or within three business days of the County's written request, to maintain eligibility for local preference consideration. Vendors seeking to qualify for the location tiebreaker must also provide a copy of their Broward County local business tax receipt by the solicitation closing date.

The form establishes five options for vendor classification: Local Business only, Locally Based Business, Locally Based Subsidiary, Joint Venture composed of qualifying entities, or non-local vendor status. Each classification requires specific certifications regarding continuous physical presence in Broward County for at least one year preceding the bid posting date, zoning compliance, operational control from the local business location, and employee primary work address alignment. Vendors must attest to the accuracy of all submitted information under penalty of potential suspension, debarment, contract termination, and civil or criminal penalties for false or fraudulent submissions. The County retains the right to request additional documentation including proof of property ownership or use rights, corporate organizational charts, joint venture agreements, and other location-related verification materials to support eligibility determinations.
