About this file
The document is a VA-FSC Vendor File Request Form (VA Form 10091), which is used to gather essential payment and vendor registration information for the Department of Veterans Affairs. The form is designed for new and existing vendors to provide critical details for establishing or updating electronic payment records, including banking information, contact details, and unique identifiers such as the Unique Entity Identifier (UEI) and Employer Identification Number (EIN).
The form includes digital instructions introduced in November 2024, requiring vendors to register with id.me to access the Customer Engagement Portal (CEP) and complete the digital version of the form. Commercial vendors must be registered in the System for Award Management (SAM) to access payment history and full portal features. The form applies to various vendor types including commercial entities, federal agencies, individuals, and VA employees, and is part of the VA's efforts to streamline vendor onboarding and payment processes while enhancing security measures.

VA-FSC Vendor File Request Form (VA Form 10091)- Digital Instructions
The Financial Services Center (FSC) has introduced a digital version of the VA Form 10091 (VA-FSC Vendor File Request Form) that is available through the FSC’s Customer Engagement Portal (CEP).
Please note that vendors (Commercial, Individual, and VA employee) must first register with id.me to take advantage of the new digital VA Form 10091 you can click on the CEP link at https://www.cep.fsc.va.gov/; This will take you to the appropriate site for id.me to start the webform process.
Commercial Non-System of Award Management (SAM) vendor accessing CEP is limited to ONLY the digital VA Form 10091. Non-SAM Commercial Vendors will NOT be allowed to access the CEP and will not have access to obtain reports or check on payments in CEP if they are not actively registered in the SAM.
VA encourages all Non-SAM Commercial Vendors to register for a SAM account at the following https://sam.gov//.
SAM registration provides additional security protections against security risks of potential fraudsters. It also minimizes the need to call the Financial Services Center’s Help Desk for payment history or statuses.
The digital form does not change how and when the VA Form 10091 is used. Instead, it provides our customers another method to complete the vendorization process if the user is registered through ID.me.
If vendors (Commercial and Individual) have questions, contact VA Customer Care Center at 1-877-353-9791 option 2 or vafsccshd@va.gov.
