PAST PERFORMANCE QUESTIONNAIRE
Unarmed Security Guard Service at Statue of Liberty National Monument & Ellis Island
(Name of Firm Submitting a Proposal), an offeror on the subject solicitation, requests your participation in responding to this questionnaire:
The purpose of this questionnaire is to obtain information regarding the quality and extent of the above-named contractor's past performance relative to a contract, completed or in progress, for your company/agency.
This contractor has submitted a proposal for the following work:
Unarmed security guard services at the Statue of Liberty National Monument New York, NY and Ellis Island Jersey City, NJ (STLI) of the Interior Region 1 – North Atlantic - Appalachian. The contractor shall perform all security guard services to provide security at the Marine Inspection Office (MIO) Parking Area and Docks, the Ellis Island Bridge, the Statue of Liberty Screening Site, Liberty State Park Screening Site, Battery Park Screening Site and Battery Park Security Post, and to monitor all security systems, which include interior and exterior intrusion detection, entry access control systems, fire alarm, and closed circuit television surveillance systems, and to provide as needed service for Special Events.
We would greatly appreciate your input regarding the Contractor's past performance.
Handwritten responses are sufficient. We request that the individual(s) responsible for the administration oversight of the project (e.g., Project Manager, Contracting Officer's Representative (COR), etc.) respond to this questionnaire.
