NOTE: The floor in the DFAC of Bldg 101 has tile placed over what is known to be Asbestos Tile. This was discovered during a remodel contract.

The records have disappeared. This information is coming from interviews done with people that were working that project.

This represents a building that had been demolished.

The new Bldg 109 was built in 2006. Due to the date we do not anticipate any ACM to be found in the new building.

MEMO TO RECORD:

SUBJ: Analysis of Suspect Asbestos Material from Bldg 101

1. As per the attached Journal entry, some pipe joints in Building 101 have insulation that appears different from the insulation on the straight runs of pipe. That joint insulation to be a form that might contain asbestos. The suspect joint insulation was the black wrapped joints.

2. Sometime during the week of 5 January 2004 SMS Simpson brought me a section of piping (see drawing below) that contained the suspect joint insulation. On 14 January I provided the piping to Donna McNeal, Orion Environmental Services. They were to sample the suspect material and analyze it for the presence of asbestos. On 21 January I received their analysis of the material. Their analysis showed no asbestos detected.

During a remodel to the DFAC the contractor found Asbestos tile. No abatement was performed. New tiles were placed over the ACM tiles. See Note on pg 6 of the Minot survey (this attachment)
