Statement of Work

BUILDING 6 RENOVATION – PHASE IV

CAJMTC DPW #2020-001

INTRODUCTION

1. Project Number and Title: (DPW #2020-001) B6-RENO_PH-IV

2. Facility Function: This Bid Build project consists of improvements that are required for renovation to southernmost wing of the Regional Training Institute (RTI). Building 6 is located behind Building 5 which is south of Headquarters Road within the cantonment area at Camp Atterbury Joint Maneuver Training Center (CAJMTC). This project is situated in Johnson County, Nineveh Township, Camp Atterbury, Indiana (CAIN) and serves as billeting quarters for transient training.

The project duration period of performance is 270 calendar days. This is a lump sum lowest responsible bid contract, and award will be subject to availability of funds.

3. Scope: The purpose of this Statement of Work (SOW) is to provide Bid Build services for the scope of work and is provided to convey the OWNER’s intent, when used in conjunction with the Construction Drawings and Technical Specifications as well as applicable Code Compliance, which provides a comprehensive document to be used for contractual bidding, and construction operations. This project requires the CONTRACTOR to provide supervision, coordination, materials, tools, equipment, and labor required to complete the following work.

a. Phase 4: No exterior work is included in this phase. Interior work includes work on the first and second floor in the West wing of the building. Demolition work in the identified Room Groups Types “A”, includes selected walls, resilient floor tile, ceramic tile, ceilings, plumbing systems, selected fire protection systems and selected electrical systems, including replacement of the fire alarm system and all lighting fixtures. Demolition work in corridors 1-W and 2-W includes the flooring and wall base. New construction includes corridors 1-W and 2-W finishes as well as work in the identified Room Groups Type “A” . Work in Room Groups Type “A” includes selected walls, ceilings, flooring, doors, door frames, casework and signage. New mechanical and electrical work includes removal of selected plumbing fixtures, revision of bathroom exhaust system components and selected electrical system components, including replacement of lighting fixtures. See the drawings for additional Phase 4 work.

Note: Balance of the facility that is not part of Phase IV will remain occupied by the client for billeting purposes. The CONTRACTOR will be required to coordinate with the RTI

Facility Manager for agreed upon hours of construction operations, as well as prior contact with twenty-four notice to coordinate performance of work that will entail considerable noise levels.

4. Project Overview: The CONTRACTOR will supply all labor, materials and equipment to:

a. Notify Facility Manager of any remaining items considered Government Property left in vicinity of Phase IV prior to commencement of the Project.

b. Prepare Temporary Protection, and Maintain During Construction Sequences.

c. Perform Selective Demolition for Performance of Work applicable Disciplines.

d. Perform remediation in areas as identified in the Contract Documents.

e. Furnish and Install New Sanitary Piping.

f. Replace Portions of Concrete Slab on Grade.

g. Perform Interior Build-Out.

h. Furnish and Install New Plumbing.

i. Retrofit Existing and Provide New Exhaust.

j. Retrofit Existing and Provide New Electrical.

k. Prep Existing and Provide New Doors/ Hollow Metal Frames

l. Repair Existing Substrates and Prepare Interior Finishes.

m. Provide Final Finishes.

n. Install New Lighting.

o. Provide New Door Hardware.

p. Install Government Furnished Materials : Plaques as shown on Sheet A106 - Signage Plan

q. Perform Final Cleaning

r. Compile Final Close-out Documents

5. References: work shall be in accordance with (IAW) the most current edition of all applicable codes and standards including but not limited to:

6. General Project Requirements: Unless otherwise indicated, this is to be designed in accordance with and adherence to the Project Specifications and Drawings, all Federal, State, local laws; regulations, rules and codes, to include OSHA, IOSHA, AFOSH, and to abide by all installation policies, guidelines and procedures including, but not limited to, the general contract provisions given below. In case of conflict between this SOW and references, the contractors bidding the work are to forward the conflict to the Contracting Officer for resolution.

7. Attachments: Work will be predicated upon government furnished information, provided to the CONTRACTOR for construction purposes. The CONTRACTOR shall receive the requisite information from the OWNER consisting of the following.

8. Submittals: The CONTRACTOR shall prepare submittals using form AF 3000 in accordance with the AF 66 Form provided, as identified within the itemized subsections in the Statement of Work, and as required by specifications. Digital files of drawings to be electronically transmitted in .pdf or .dwg format, and product literature, technical specifications or supplemental data to be submitted in .pdf or MS Word format. Hard copies of submissions are acceptable providing three sets are transmitted with the AF3000. Digital drawing files in AutoCAD may be requested through DPW COR/COTR if required

9. General Contract Provisions: The CONTRACTOR shall be required to adhere to installation standard policies as described below:

Environmental/Cultural Issues:

a. The CONTRACTOR must comply with all cultural resource laws and regulations, including the National Historic Preservation Act and the Archaeological Resource Protection Act. If any discovery of an archaeological artifact or features such as but not limited to structure foundations, pier stones, old glassware and ceramic ware, old building remnants, Indian arrowheads, Indian axe heads (adzes), stone tools, crock ware, metal objects, etc. or human remains while performing the work outlined here the CONTRACTOR will immediately cease all work within 30 yards of the area of the discovery and contact the CA-DPW-ENV. Ground disturbance may only resume under the direction of the CA-DPW-ENV.

b. The CONTRACTOR shall be responsible for obtaining and complying with NPDES, Rule 5 Erosion control permits. The CONTRACTOR will obtain all required permits prior to the initiation of construction. All fees associated with permitting shall be at the CONTRACTOR's expense.

c. No open fires will be allowed without coordination with the CA-DPW-ENV.

Construction and Demolition:

a. Construction and Demolition: Excess material resulting from excavation, demolition, and construction which is not identified to remain or be salvaged, shall be removed from the installation and relocated by the CONTRACTOR. At a minimum, recyclable construction and demolition debris that are taken offsite shall be diverted from the non-hazardous solid waste stream (reused/recycled) IAW IC 13-20-25 and Executive Order 13514.

b. Environmental Requirements: The CONTRACTOR will be in compliance with all Federal, State, and Local laws, regulations, policies and procedures to include air, water, and solid waste requirements, and shall research and prepare necessary permit applications.

Effective 23MAR2016 Atterbury-Muscatatuck has adopted the requirements set forth in FAR 52.223-9 and 52.223-10. The CONTRACTOR shall perform demolition work that complies with this waste reduction initiative, maintain accurate logs related to construction debris, and assemble associated documentation required for close-out procedures on for this project.

All disposal weights shall be provided by the CONTRACTOR to DPW COR via tipping fees or disposition receipts throughout the course of the project. Copies are to be maintained by the CONTRACTOR and log to assemble final tabulation at the end of the project.

Contractor Manpower Reporting The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the National Guard Bureau (NGB) via a secure data collection site. The contractor shall completely fill in all required data fields using the following web address: https://www.sam.gov Reporting inputs shall be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk at https://www.sam.gov .
