About this program

The Centers for Disease Control and Prevention (CDC) within the Department of Health and Human Services funds chronic disease prevention and control assistance programs (CFDA 93.945) through cooperative agreements and project grants, with the most recent award issued September 30, 2026.

The program supports planning, development, integration, coordination, and evaluation of chronic disease prevention and control initiatives with state health agencies and nonprofit organizations, with emphasis on monitoring behavioral risk factors associated with the ten leading causes of premature death and disability in the United States—particularly cardiovascular disease, diabetes, obesity, and stroke. Recent awards have funded initiatives ranging from mobile interventions targeting physical activity in older cancer survivors to wearable devices for maternal hemorrhage monitoring, surveillance networks for diabetes in youth and young adults, prevention research centers addressing health equity, school-based adolescent health surveillance, and culturally tailored chronic disease prevention programs serving American Indian and Alaska Native populations. Cooperative agreements dominate the portfolio at 57 percent of awards (415 awards), with project grants comprising 38 percent (278 awards) and block grants and formula grants together making up the remainder.

All funding flows through CDC; the program obligated $2.21 billion in combined federal dollars across 724 awards since 2002. Award values vary widely, from under $300 thousand for school surveillance to over $14 million for comprehensive state obesity and cardiovascular health initiatives. The program dispersed 116 awards in 2023, 53 in 2024, and 8 awards through September 2026, indicating declining recent activity. Cooperative agreement performance periods extend up to five years with renewal possible on a non-competitive basis contingent on satisfactory progress.

Recently posted opportunities include a forecast for chronic disease education and awareness (CDC-RFA-DP-27-0068, posted October 9, 2026, award range $325,000 to $375,000) and breastfeeding support and hospital quality improvement (CDC-RFA-DP-27-0063, posted September 25, 2026). State health agencies, tribal organizations, federally recognized tribal governments, counties, nonprofits, universities, and community-based organizations are eligible. Applicants must comply with 2 CFR 200 and maintain records for three years beyond audit completion.
