About this opportunity
The Centers for Disease Control and Prevention is soliciting applications for cooperative agreements to strengthen national chronic disease education and awareness for chronic diseases not supported through disease-specific CDC funding, with no published application due date listed in this forecast. Awards range from $325,000 to $375,000. CDC will deem non-responsive and reject any application proposing more than one chronic disease, a disease not meeting CDC's definition (a condition lasting 1 year or more requiring ongoing medical attention or limiting activities of daily living), or any disease with dedicated FY 2027 funding (Arthritis, Heart Disease, Cancer, Stroke, Alzheimer's Disease and Related Dementias, Diabetes, Epilepsy, Lupus, and Chronic Kidney Disease).
Eligible applicants include higher education institutions (public and private), small businesses, nonprofits (501(c)(3) and non-501(c)(3)), independent school districts, county and state governments, city and township governments, tribal organizations and governments, public or Indian housing authorities, special district governments, for-profit organizations, and bona fide agents. Applicants must upload a PDF statement identifying the proposed chronic disease under miscellaneous attachments, named "Proposed Chronic Disease."
Recipients will review and disseminate surveillance data and evidence-based strategies; develop or refine survey tools to improve assessment and surveillance; create and disseminate educational resources for public health professionals, health care professionals, and the public; and strengthen partnerships expanding reach and use of evidence-based information. The program aims to improve access to evidence-based education and awareness resources, strengthen surveillance and dissemination of public health information, and improve awareness and use of evidence-based practices.
The underlying program (CFDA 93.945, Assistance Programs for Chronic Disease Prevention and Control) supports cooperative agreement periods of 1 to 5 years, with non-competing continuation possible subject to satisfactory progress and federal interest.
Forecast synopsis
This NOFO will support organizations to strengthen national chronic disease education and awareness for chronic diseases that are not supported through disease-specific CDC funding by implementing activities that enhance surveillance, address priority evidence gaps, develop and disseminate evidence-based educational resources, strengthen strategic partnerships, and promote the adoption of evidence-based prevention, screening, referral, treatment, disease management, and self-management practices.
Recipients will review and disseminate surveillance data and evidence-based strategies, develop and/or refine survey tools to improve assessment and surveillance, create and disseminate educational resources for public health professionals, health care professionals, and the public, and strengthen partnerships that expand the reach and use of evidence-based information.
This NOFO is expected to improve public health by increasing access to evidence-based education and awareness resources, strengthening surveillance and dissemination of public health information, improving awareness and use of evidence-based practices among public health professionals, health care professionals, and the public, and supporting improved health outcomes for people living with chronic diseases.
Eligibility
Bona fide agents are eligible to apply. For more information about bona fide agents, please see the CDC webpage on Expediting the Federal Grant Process with an Administrative Partner.CDC will not consider any application that proposes: 1) More than a single chronic disease to focus on during the life of the cooperative agreement; 2) A proposed chronic disease that does not meet the definition of a chronic disease. A chronic disease is defined broadly as a condition that lasts 1 year or more and requires ongoing medical attention or limits activities of daily living or both; or 3) Arthritis, Heart Disease, Cancer, Stroke, Alzheimer's Disease and Related Dementias, Diabetes, Epilepsy, Lupus, and Chronic Kidney Disease, or any other chronic disease with a dedicated funding line in the FY 2027 Appropriations Act, as described on CDC's FY 2027 operating budget plan. Any application proposing a chronic disease that meets any of these three (3) criteria will be deemed non-responsive, and it will receive no further review. Applicants must upload a statement that identifies their proposed chronic disease as a PDF file, under miscellaneous attachments, and name the file "Proposed Chronic Disease." The award ceiling for this NOFO is $375,000. CDC will consider any application requesting an award higher than this amount non-responsive, and it will receive no further review. The award floor for this NOFO is $325,000. CDC will consider any application requesting an award lower than this amount non-responsive, and it will receive no further review.
