About this opportunity

The State of West Virginia Department of Health and Human Resources, Bureau for Behavioral Health and Health Facilities, is soliciting competitive quotations for pest control services at Hopemont Hospital located in Terra Alta, West Virginia. This Request for Quotation (RFQ No. ARFQ HOP15*9) seeks a qualified vendor to provide comprehensive monthly pest control services and non-routine pest control visits for a 12-month contract period commencing July 1, 2015. The scope of work includes monthly pest control treatments for rodents, roaches, bees, and ants across all hospital buildings during operating hours (Monday through Friday, 8:00 a.m. to 4:00 p.m.), with services required in kitchen facilities, cafeteria areas, resident areas, elevator shafts, break rooms, physical therapy departments, activities departments, hallways, maintenance areas, receiving departments, and the clubhouse. Additionally, vendors must conduct comprehensive facility-wide treatments every six months and supply up to 50 mouse traps annually while maintaining a designated contract manager available during normal business hours. Interested vendors are required to attend a pre-bid meeting on April 22, 2015, at 1:00 p.m. at Hopemont Hospital; submit written questions by noon on April 24, 2015; and submit sealed bids by May 1, 2015, at noon. Vendors must be licensed by the West Virginia Department of Agriculture for pesticide application and provide evidence of Commercial General Liability Insurance ($1,000,000 minimum) and Workers' Compensation Insurance. Award will be made to the vendor submitting the lowest overall lump sum cost that meets all specifications, with pricing submitted on the required Pricing Page form. Bids may be submitted through wvOASIS, fax, courier, USPS, or hand delivery to Shelby Hauser at the Hopemont Hospital Accounts Payable office (304-789-2411, extension 277; fax 304-789-2233; shelby.a.hauser@wv.gov).

West Virginia Code 5A-3-37 permits qualified vendor preference designations for eligible resident vendors and businesses. Vendors meeting residency requirements (four continuous years of West Virginia residency or business location) may qualify for up to 2.5 percent preference, with additional 2.5 percent preference available if at least 75 percent of project employees are West Virginia residents. Veterans may qualify for 3.5 percent preference under specified conditions, and non-resident small, women- and minority-owned businesses certified by the Purchasing Division may apply for preference as well. All vendors responding to this solicitation must submit a notarized Purchasing Affidavit (WV Code 5A-3-10a) confirming that neither the vendor nor related parties have outstanding state debts exceeding $1,000 or employer default status. Vendors must also certify a Vendor Preference Certificate at the time of bid submission if claiming eligibility for residency-based preference. The contract incorporates the West Virginia Purchasing Division's Master Terms and Conditions by reference and is governed by West Virginia law. No incumbent vendor or current contractor information is specified in the solicitation documents. The contract award value is not explicitly stated; however, pricing will be based on a flat monthly fee for 12 months of service plus per-visit charges for non-routine pest control calls, with payment made monthly in arrears following service delivery and acceptance. The contract term is for an initial one-year period with an option for up to three successive one-year renewal periods, not to exceed 36 months total.
