About this file

This is a Purchasing Affidavit form issued by the State of West Virginia Purchasing Division for pest control services (RFQ No. ARFQ HOP15*9). The document serves as a compliance certification requirement for all vendors responding to state and local contract opportunities in West Virginia. The affidavit mandates vendor attestation regarding financial and employment obligations prior to contract award, with no specific information provided regarding project scope, quantities, response deadlines, or contract term in this particular document.

The affidavit establishes strict eligibility requirements under W. Va. Code 5A-3-10a, prohibiting contract awards to vendors or related parties with outstanding debts exceeding one thousand dollars in aggregate or those in employer default status. Acceptable exceptions include contested tax matters not yet finalized and vendors in compliance with established payment plans. The form requires notarization and the vendor's authorized representative must affirm under penalty of law that neither the vendor nor any related parties meeting the definition of ownership or interest (five percent or greater of total contract value) are subject to the disqualifying debt or employer default conditions.

MANDATE: Under W. Va.
