About this file
This is a Request for Quotation (RFQ) issued by the West Virginia Department of Health and Human Resources, Behavioral Health and Health Facilities division, for Hopemont Hospital to establish a contract for monthly pest control services. The solicitation (ARFQ HOP150000009) requires vendors to provide labor, equipment, and materials for monthly pest control services and non-routine visits for rodents, roaches, bees, and ants across all buildings owned by Hopemont Hospital. Services must be performed Monday through Friday between 8:00 a.m. and 4:00 p.m., with treatment required in specified areas including kitchen facilities, cafeteria areas, resident areas, elevator shafts, break rooms, physical therapy departments, and activities departments. Additionally, vendors must conduct comprehensive facility-wide treatments every six months covering hallways, maintenance areas, receiving departments, and the clubhouse. Vendors must supply up to 50 mouse traps per contract year and maintain a designated contract manager available during normal business hours. Vendors must be licensed by the West Virginia Department of Agriculture for pesticide application and perform services with an escort from the facility's Housekeeping Supervisor.
Award will be made to the vendor providing the required contract services at the lowest overall total cost based on a flat monthly fee submitted on the Pricing Page. Payment will be made monthly for all services performed and accepted, with vendors responsible for all mileage and travel costs incorporated into the flat fee bid. The contract will be immediately cancelable for vendor default, including failure to perform services according to specifications, failure to comply with applicable laws and regulations, or failure to remedy deficient performance upon request. Vendors should submit their proposals, including completed pricing pages, through wvOASIS or as electronic documents by emailing requests to Shelby.a.hauser@wv.gov.
