About this opportunity

The State of Utah Department of Health and Human Services is seeking qualified professional service providers and consultants through a Request for Statements of Qualifications (RFSQ) for the MK26-21 Chronic Disease Prevention Partnership, a five-year contract under Utah Procurement Code 63G-6a Part 15. The procurement targets vendors capable of providing training and technical assistance to selected Health Centers and affiliated healthcare organizations in tobacco cessation and breast and cervical cancer screening interventions. Primary deliverables include training on tobacco cessation medications for uninsured populations, screening and cessation interventions, an adolescent vaping cessation pilot project, quarterly and annual surveillance data reviews, communication liaison services with local health departments, baseline enrollment reports due by May 30th annually, and annual data reports due by August 30th annually. Key performance objectives require 500 referrals to Utah Tobacco and Nicotine Quit Services, 100 Health Center patients receiving tobacco cessation medications, 87 percent of tobacco-using patients receiving cessation interventions, and evidence-based intervention assistance to 13 participating Health Systems for both breast and cervical cancer screening. Mandatory minimum requirements for vendor eligibility include at least five years of experience implementing evidence-based interventions within health centers, access to health center reporting systems, demonstrated ability to serve medically underserved and high-need populations including rural and frontier areas, capacity to provide training and ongoing support to health center staff, and commitment to quarterly and annual reporting. Vendors must submit statements of qualifications without pricing information by the April 1, 2026 deadline, and will be evaluated against five weighted technical criteria totaling 875 possible points with a mandatory 65 percent minimum threshold (568.8 points) to advance; the highest-ranked vendor will be selected for fair and reasonable compensation negotiations.

No specific set-asides or organizational restrictions limiting participation to disadvantaged enterprises, minority-owned businesses, women-owned businesses, or other protected classes are identified in the procurement documents. The solicitation does not identify any incumbent vendors or current contractors, nor does it reference existing infrastructure or technology systems requiring vendor integration beyond the requirement that contractors must have access to health center reporting systems for baseline and annual data collection. The state reserves the right to expand contract services to include the Healthy Environments, Active Living (HEAL) program at any time during the five-year contract term through signed amendments. The annual contract budget is $130,000, with approximately $85,000 allocated to the Tobacco Prevention and Control Program and $45,000 designated for the Utah Breast and Cervical Cancer Program across five fiscal periods running from July 1, 2026 through June 30, 2031, resulting in a total contract value of $650,000 over the full term. Vendors responding to this solicitation must comply with standard terms and conditions established by the Utah Department of Health and Human Services, including maintenance of minimum $1,000,000 general liability insurance per occurrence, compliance with federal and state laws, E-Verify registration, and strict data security and confidentiality protocols for protected health information.
