About this file

This is a Scope of Work document for the State of Utah Department of Health and Human Services' Chronic Disease Prevention Partnership contract (MK26-21), a five-year professional services agreement designed to provide training and technical assistance to Health Centers and affiliated health care organizations. The contract encompasses two primary programs: the Tobacco Prevention and Control Program (TPCP) and the Utah Breast and Cervical Cancer Program (B&C). Under TPCP, the contractor shall provide training on tobacco cessation medications for the uninsured, screening and cessation interventions, and an adolescent vaping cessation pilot project; conduct quarterly and annual surveillance data reviews; serve as a communication liaison with local health departments; and submit quarterly and annual progress reports. The B&C component requires the contractor to evaluate screening programs using evidence-based interventions, analyze annual screening rates, assess training needs, provide best practices for optimizing clinical workflows, implement quality improvement processes, deliver patient education and outreach, and submit baseline enrollment reports by May 30th annually and annual data reports by August 30th annually.

Funding is allocated at $85,000 annually for TPCP and $45,000 annually for B&C across five fiscal periods running from July 1, 2026 through June 30, 2031, totaling $650,000 over the contract term. The TPCP program has specific performance outcomes requiring 500 referrals to Utah Tobacco and Nicotine Quit Services, 100 Health Center patients receiving tobacco cessation medications, and 87 percent of tobacco-using patients receiving cessation interventions. The B&C program aims to increase evidence-based intervention assistance to 13 participating Health Systems for breast cancer screening and 13 for cervical cancer screening. The state reserves the right to add additional services, including the Healthy Environments, Active Living (HEAL) program, at any time during the contract term through a signed amendment.
