About this file
This document is a mandatory minimum requirements and technical scoring criteria attachment for a state and local contract opportunity issued by the State of Utah Department of Health and Human Services. The MK26-21 Chronic Disease Prevention Partnership is a five-year professional services contract for providing training to selected health centers and affiliates on tobacco cessation and breast and cervical cancer screening evidence-based interventions. The request for statements of qualifications (RFSQ) seeks vendors capable of implementing these interventions within health centers and serving medically underserved and high-need populations throughout Utah, including rural and frontier areas. The contract may be expanded to include additional services such as the Healthy Environments, Active Living (HEAL) program at any time during the contract term through signed amendments. No specific response dates, due dates, site visits, or bidder conferences are identified in this attachment.
To advance to technical criteria evaluation, vendors must satisfy five mandatory minimum requirements: demonstrate at least five years of experience implementing tobacco cessation and cancer screening evidence-based interventions within health centers; have access to health center reporting systems for baseline and annual data collection; possess the ability to reach medically underserved populations with limited healthcare access; provide training, education, and ongoing support to health center staff; and commit to submitting quarterly and annual reports. The annual contract budget is $130,000, with approximately $85,000 allocated to tobacco cessation programs and $45,000 for cancer screening initiatives. Vendors selected for award will be those with the highest ranking following technical evaluation, subject to negotiation of fair and reasonable compensation. If negotiations fail with the highest-ranked vendor, the state may proceed with the next highest-ranked vendor.
