About this file
This is an evaluation scoresheet for a Request for Statement of Qualifications (RFSQ) issued by the State of Utah Department of Health and Human Services for the Chronic Disease Prevention Partnership (Solicitation #MK26-21). The contract seeks a vendor to provide training to selected health centers and affiliates on tobacco cessation and breast and cervical cancer screening evidence-based interventions. The contract term is five years, with the state reserving the right to add additional services, including the Healthy Environments, Active Living (HEAL) program, at any time during the contract period through signed amendments. No pricing information is to be included in vendor submissions, as this is a qualifications-based selection process under Utah Procurement Code 63G-6a Part 15 for design professional service providers and consultants.
The evaluation scoresheet establishes five mandatory minimum requirements that vendors must satisfy on a pass/fail basis, including at least five years of experience implementing relevant evidence-based interventions within health centers, access to health center reporting systems, ability to serve medically underserved and high-need populations, capacity to train and support health center staff, and willingness to submit quarterly and annual reports. The scoresheet contains five scoreable technical criteria weighted between 33 and 38 points each, for a total of 875 possible points, with a required minimum threshold of 568.8 points (65 percent) to advance. The highest-ranked vendor will be selected to negotiate a contract at fair and reasonable compensation as determined by the state; if negotiations fail with the top-ranked vendor, the state may proceed with the next highest-ranked vendor.
