About this file

This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees bidding on state contracts, specifically for the procurement of a Roland XG-640 Printer/Cutter for Correctional Industries Huntingdon. The certification form mandates that all bidders attest to compliance with Pennsylvania's labor and workforce safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and twelve additional state labor statutes. Bidders must complete and submit this form as an attachment to their bid response through the Pennsylvania Supplier Portal at https://pasupplierportal.state.pa.us/irj/portal/anonymous. The solicitation is electronic bid only, and interested vendors must register at www.pasupplierportal.state.pa.us to obtain a vendor number and submit their bids; machine specifications must also be included with all bid responses. Award will be made to the lowest responsible bidder based on total bid price, with all items delivered F.O.B. Destination at the supplier's expense.

The certification form requires an authorized company official to sign and verify compliance status, with the understanding that false certifications subject the contractor to penalties including contract termination and three-year ineligibility to bid on future Commonwealth contracts under 62 Pa. C.S. § 531. Certifying officials must also agree to immediately report any changes in compliance status to the purchasing agency and acknowledge that the certification is subject to the provisions and penalties of 18 Pa. C.S. § 4904 for unsworn falsification to authorities. All bidders are responsible for monitoring the DGS emarketplace website for change notices, which become incorporated into the IFB. Vendors requiring assistance with the registration or bidding process may contact the helpdesk at (877) 435-7363, option 2.
