About this file
This is a Performance Assessment Questionnaire used by the US Army Contracting Command – Aberdeen Proving Ground to evaluate offerors' past performance in support of RFP W91CRB-26-R-A006. The questionnaire is sent to identified points of contact who have worked with prospective contractors to provide candid assessments of their performance on recent and relevant federal, state, local government, or commercial contracts comparable to NAICS 339113.
Respondents must complete the questionnaire and return it to Andrea Seaman (Andrea.L.Seaman.civ@army.mil) and Shelby Saum (Shelby.a.saum.civ@army.mil) no later than 15 days after receipt or April 21, 2026, whichever occurs first. The questionnaire is structured in three parts: Part I requires the offeror to provide contract identification information including contractor name, address, program title, contract number, contract type, award date, completion date, contract value, and the offeror's representative details. Part II requires the responding point of contact to evaluate the contractor across five performance criteria—compliance with specification requirements and workmanship, project management effectiveness, timeliness of performance, cost forecasting and control (for cost reimbursement contracts only), and commitment to customer satisfaction—using four confidence levels (Substantial, Satisfactory, Limited, or No Confidence) with required rationales for each response. Part III provides instructions to return the completed questionnaire to the identified Contracting Officer. The assessment is designated as source selection information under FAR 2.101 and 3.104.
