About this file
This is a PM SPE Engineering Change Request (ECR) form template used by vendors to request modifications to federal contracts and procurement activities.
The form requires vendors to complete blocks 2-19 and submit to PM SPE. Key information fields include: originator details (name and address); ECR type classification as ECP, RFD, or RFW; severity classification (Critical, Minor, or Major); ECR title; item designation (MCN/NSN and model/design code); procurement activity and baseline affected (Functional, Product, or Allocated); end item nomenclature; contract number and line item; lots/materials affected; reoccurring deviation status with CAR number if applicable; effects on cost/price and delivery schedule; other affected systems/configuration items/interfaces; detailed description of the change; need justification; and corrective actions taken. The submitting activity section requires typed name, title/organization, signature, and date in YYYYMMDD format.
The approval workflow varies by ECR classification: Minor ECRs require Contracting Officer Representative (COR) approval; Major ECRs require both COR/PM SPE Technical Staff approval and Assistant Program Manager (APM) approval; Critical ECRs require approval from all three levels plus PM SPE. Each approval section includes spaces for approval/disapproval selection, signature, and date. The form also includes a PM SPE Justification section (Block 20) and Attachments section (Block 21) for supporting documentation. This is a three-page template with space for continuation of narrative sections on pages 2 and 3.
