About this file

This is a Performance Assessment Questionnaire used by the U.S. Army Contracting Command – Aberdeen Proving Ground to evaluate offeror past performance in support of RFP W91CRB-25-R-0003. The questionnaire is issued by Contracting Officer Shelby A. Saum and must be completed by points of contact who have direct knowledge of an offeror's performance on recent and relevant federal, state, local government, or commercial contracts comparable to NAICS code 339113. Completed questionnaires must be returned to Andrea Seaman and Shelby Saum no later than 15 days after receipt or November 7, 2025, whichever occurs first.

The questionnaire consists of three parts. Part I requires the offeror to provide contract identification information including contractor name, address, program title, contract number, contract type, prime contractor name if applicable, award date, completion date, nature of contractual effort, and total contract value, along with identification of the offeror's representative. Part II, completed by the respondent point of contact, evaluates performance across five key areas using a four-tier confidence rating system (Substantial Confidence, Satisfactory Confidence, Limited Confidence, or No Confidence): compliance of products and services to specifications and workmanship standards; effectiveness of project management including subcontractor control; timeliness of performance; effectiveness in forecasting and controlling costs for cost reimbursement contracts only; and commitment to customer satisfaction. Each rating requires supporting rationale. The questionnaire also requests general comments, identification of other relevant past efforts with alternative points of contact, and respondent identification information. This is a template document designed to collect standardized performance feedback from previous customers of potential contractors.
