STATEMENT OF WORK

FOR

DESIGN AND CONTRUCTION OF:

2018 POL EFSO Repair

May 2018

Part 1 GENERAL REQUIREMENTS

1.1. SCOPE OF WORK

A. Briefly and without force and effect on the requirements of the Contract Documents, the project and the work of the Contract is described in summary, in “Part 2, Job Description.”

1.2. OBLIGATION OF CONTRACTOR

A. The Contractor shall furnish, under this contract, all materials, labor, tools and equipment necessary to complete all work specified herein. The Contractor shall so arrange and supervise his work force as to perform all work in accordance with all applicable codes and regulations and in a manner satisfactory to the Contracting Officer. The Contractor shall remove all surplus material, debris and construction equipment from around all construction areas and shall leave his work, at the end of working periods, in a condition satisfactory to the Contracting Officer.

The Contractor shall provide his own telephone if required.

B. Access and General Protection/Security Policy and Procedures. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DoD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.

C. For Contractors that do not require CAC, but re quire access to a DoD Facility and/or Installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

D. AT Level I Training. All contractor employees, to include subcontractor employees, requiring access Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training within 31 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Certificates of completion for each affected contractor employee and subcontractor employee will be maintained by the COR or Antiterrorism Representative. E. Project Management Software. The Contractor shall use project management software ‘Gate Three’ for all submittal documents. The government shall provide login credentials and instructions for submittals.

1.3. MAINTENANCE OF ADJACENT USE

A. The contractor shall conduct his work in such a manner as to not interfere with or endanger work or traffic in areas adjacent to the construction areas, except as permitted by the Contracting Officer. The Contractor shall so arrange his construction operations as to provide access for emergency vehicles and equipment to the work site at all times.

1.4. WORK SCHEDULE

A. The Contractor shall notify the Contracting Officer’s Representative at least one day prior to the commencement of work.

B. If any contractor personnel or sub-contractors are not badged, it is the responsibility of the badged contractor to escort any un-badged personnel working on the base.

C. Work is permitted on the base Monday through Friday, 7:00 a.m. to 4:30 p.m. Waivers of this requirement may be considered on a case-by-case basis, solely at the option and convenience of the Government. No waiver of the requirement shall in any way limit the Government’s right to require future performance between the hours of 7:00 a.m. and 4:30 p.m., Monday through Friday. No work is permitted on the Base on the following holidays without prior coordination:

New Year’s Day Christmas Day Columbus Day Memorial Day President’s Day Veteran’s Day Thanksgiving Day Independence Day

1.5. SAFETY PRECAUTIONS

A. Prior to commencement of work, the Contractor shall familiarize himself with all safety regulations and practices applicable with construction operations on military property. No additional payments will be made for equipment and procedures necessitated by these safety precautions. All OSHA and VOSHA regulations apply to all contractor work performed on the base.

1.6. DISPOSAL OF DEBRIS

A. All debris and excess materials, not designated as salvage by the Contracting Officer, shall be promptly removed from Government property. The Contractor shall receive title to all debris and/or excess material not designated for salvage by the Contracting Officer. The government will not be responsible for any loss or damage to debris or excess material owned by the Contractor.

1.7. FIRE PROTECTION

A. The Contractor shall take all necessary precautions to prevent fires adjacent to the work and to his buildings, and he shall prevent the spread of fires to areas outside the limits of the work.

He shall provide adequate facilities for extinguishing fires and shall maintain a proper patrol during the burning of any materials under this contract to prevent the spread of fires beyond their proper limit. Compliance with the 158 Fighter Wing Reg. 92-1, Contractor's Guide and Air National Guard Regulation 92-1 is mandatory. Disposal of debris by burning will not be allowed.

1.8. ENVIRONMENTAL CONDITIONS

A. Contractor will be in compliance with all applicable State and Federal environmental Regulations. Contractor shall provide all Material Safety Data Sheets (MSDS) on all hazardous materials being used in this project. Contractor will take all precautions to prevent any spills of oils and/or hazardous substances. The base Fire Department shall be contacted in case of a spill.

B. Information shall be provided by the Contractor for hazardous materials being delivered onto the Vermont Air National Guard Base, including, but not limited to paints, solvents and glues to the HAZMART Pharmacy Manager located in Building 75. The POC for this information is MSgt Ian Fortier who may be reached at 802-660-5945 or via email at ian.m.fortier.mil@mail.mil. In addition to the types and quantities of chemicals, the time period chemicals are on base must be specified.

1.9. SECURITY REQUIREMENTS

A. The Contractor, contractor’s employees and sub-contractors shall all complete the BASE

ACCESS BACKGROUND CHECK REQUEST form provided by contracting. An electronic or paper template will be made available to assist in fulfilling this requirement. All personnel have a valid driver’s license and vehicle registration to drive on the base. Contractor shall obey all speed limits. No weapons are permitted on this installation. Vehicle safety restraint belts are required while operating a vehicle on this installation.

B. Government requires a project roster before commencement of any work on base. Roster shall list all the prime contractor and subcontractor employees’ complete names and the individuals consent to a background check if VTANG security deems necessary. An electronic or paper template will be made available to assist in fulfilling this requirement.

C. Each individual will be required to sign in at B215 POL office at the beginning of each duty day and sign out at B215 POL office at the end of each duty day.

D. All workers within POL shall be held responsible to ensure ALL gates are completely closed after passing through when gaining entry or exiting, if someone sees a gate open, they shall report it to the B215 POL office immediately

E. All Badge holders and non-badge holders will need to submit their full name, last 6 of SSN, company, dates they will working in POL area to SMSgt Parsons. Badge holders will be able to escort non-badges personnel within POL. The maximum number of people that a badged worker may escort is listed on their badge.

F. At times of heightened security, the contractor may be asked to leave the government property/work site on short notice. Depending on the situation this direction could come from any of the following; the Contracting Officer, the Contracting Officer’s Representative, the Base Civil Engineer, or the Security Forces Commander. The contractor shall do so in a timely manner and notify the Contracting Officer as soon as possible.

1.10. PRE-BID SITE INSPECTION

A. It is not required that prospective bidders visit the site and examine existing conditions prior to submitting a bid. Submission of a bid shall be recognition by all parties that the bidder fully understands the nature and scope of work.

1.11. QUALITY ASSURANCE

A. Provide at least one person who shall be present at all times during the execution of the Work and who shall be thoroughly trained and experienced in removal work, and who shall direct all work performed under this Section.

B. Use only personnel thoroughly trained and experienced in the skills required for executing the required work and in operating the required equipment.

1.12. SCHEDULING

A. Coordinate with the Contracting Officer’s Representative (660-5455) and/or Civil Engineering Work control (660-5327).

B. Notify Engineer at least 48 hours in advance of the beginning of any removal activity.

C. Schedule work and operations to allow ample time for inspection.

Part 2 JOB DESCRIPTION

2.1. INTENT OF SPECIFICATION

Project shall include but not be limited to the following:

A. Provide a complete functioning “turn-key” Emergency Fuel Shutoff system including but not limited to:

1. Submit proposed conduit runs/trench locations that may differ from provided design plan.

2. Furnish and install seven new EFSO stations, supporting structures, conduit/conductors, and signage as indicated on plans.

3. Replace existing EFSO signage as indicated on plans.

4. Pull southern EFSO wiring to determine wiring path as noted on plans.

5. Cut and cap two (2) existing EFSO stations below ground, as indicated on plans.

6. Program existing master alarm panel in B215. Alarm panel shall contain labeled visual lights for two zones.

i. Zone 1 shall be for all upper POL EFSO’s

ii. Zone 2 shall be for all lower POL EFSO’s

7. Alarm panel shall activate existing audio and visual alarm(s) and shut down all pumps within the POL area when any EFSO is activated. Panel shall prohibit operation of all pumps until all EFSO pushbuttons are cleared and alarms acknowledged. Existing, functioning EFSOs already perform this function.

8. Connect existing master alarm panel to B215 existing fire alarm panel. Connections to fire alarm panel shall be installed and programmed to communicate activated EFSO(s) locations (upper or lower POL) to 158th Fighter Wing Fire Department.

9. Stabilize banks to minimize erosion during and after construction activities.

10. Repair disturbed construction areas to pre-existing conditions.

B. Instruct VTANG POL personnel on proper operation of EFSO system for future operation, clearing, testing, and troubleshooting.

Part 4 MATERIALS & SPECIFICATIONS

All above ground conduit shall be EMT to protect interior conductors.

Contractor shall carefully hand excavate in all areas where damage to underground utilities may occur by excavating with machinery.

Part 5 EXECUTION

5.1. CONTRACTOR FURNISHED SERVICES

The Contractor is responsible for all work, labor, tools, equipment, materials, scheduling, Hazardous materials permits and coordination, other permits associated with performing the work in this statement of work.
