EXHIBIT A - PERFORMANCE RELEVANCY QUESTIONNAIRE

PART 1: Prepare questionnaires in the format provided.

Provide the following information requested in this format for each of the projects/contracts being described. Prepare not more than three (3) of these documents. Contractors should select their projects carefully because the quality of the contractor’s relevant performance is more important than the quantity of projects submitted. Projects may be on going, if complete to 70% and cannot have been completed any earlier than 5 years prior to the solicitation issue date. Provide frank, concise comments regarding your performance on the contracts you identify. Use as much space as required.

D. Provide detailed description of the work performed under the contract including, but not limited to, similarity of the construction methods; size; project scope; cost magnitude and complexity of work performed to those required for the proposed effort. Also discuss similarity of the client type and similar sustainability requirements. Discuss the similarity of the location at which work was performed to the location(s) at which work will be performed.

In addition, address the following: (Use as much space as necessary)

1. Describe the specific elements of the work performed by your firm.

2. Indicate what elements of work were performed by your major subcontractors and if those same subcontractors will be used on this project. If you intend to use these subcontractors, please list name and principal areas of work they will perform. Indicate if you do not intend on utilizing any of these subcontractors.

3. Address any technical areas about this project you consider uniquely relevant to this project. Include in your discussion if project had sustainability requirements. Of particular interest is the level of recognition received. Indicate if no such requirements.

4. If you intend to use any of the same key personnel on this solicitations’ project, indicate by name and position those personnel proposed to be utilized on the referenced contract. (Project Manager, Site Superintendent, Quality Control Manager). Indicate what position they will have on this solicitation’s contract. Indicate if you do not intend on utilizing any of these same personnel.

5. Describe why you believe the work is very relevant, relevant or semi-relevant to this solicited project. Refer to section 00120 for information. (Use as much space as necessary) E. List a point of Contact for Owner/Government agency Contracting Officer and or inspector; include a current phone number and facsimile number. Email address is also desired. (This information is required here even though you will be sending Exhibit C letters and questionnaires to theses references. (Do NOT include a copy of the Exhibit Cs you sent to your references.)

END OF PART 1 - Prepare and send Exhibit C Package to the reference(s) for each of projects discussed in Exhibit A, Part 1.

PART 2: Provide the following information one time.

Additional offeror information required, but not project specific. This information need only be provided once in your submission package.

2A: Applicable to all offerors - Describe your Compliance with the requirements of FAR clause 52.219-8 Utilization of Small Business Concerns. Even if you are a Small or Small Disadvantaged Business this clause in Federal Contracts applies. It is the policy of the United States that small business concerns, veteran-owned small business concerns, service-disabled veteran-owned small business concerns, HUBZone small business concerns, small disadvantaged business concerns, and women-owned small business concerns shall have the maximum practicable opportunity to participate in performing contracts let by any Federal agency, including contracts and subcontracts for subsystems, assemblies, components, and related services for major systems. It is also policy to establish procedures to ensure the timely payment of amounts due pursuant to the terms of their subcontracts. Describe below the methods you have used to comply with this policy on prior federal contracts. Include any evidence, or a point of contact to verify this information, if available.

2B: Offerors are strongly encouraged to include with their proposals information on problems encountered on relevant projects and the specific corrective actions taken to remedy the problem(s).

2C: List any projects within the last five (5) years that have been assessed Liquidated damages. If none have been assessed, make a positive statement to that effect.

2D: List any projects within the last five (5) years that have been terminated, for any reason. Provide an explanation. If none, make a positive statement to that effect.

2E: Other Past Performance Considerations (OPTIONAL) Information, if provided, shall be limited to the period starting 5 years prior to the solicitation release date and limited to not more than four (4) pages.

a) Effectiveness of value engineering (VEC) proposals you submitted on prior contracts. (Provide a brief description; impact on cost, schedule, efficiency or quality; contract number; name and phone number of customer benefiting from the VEC).

b) Professional/Industry awards (Identify award type, date of award and copy of certificate if applicable), Letters of appreciation, recognition or commendations.

c) Discuss unique skills and accomplishments (Explain unique skills and/or accomplishments and provide supporting information for verification).

For Subcontractor, Teaming partners and/or potential key personnel Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted to the offering contractor for submission with subcontract past performance volume. Additionally certain subcontractors will be required to send off questionnaires to their references. The offering contractor will inform you if this is required. Your cooperation will be appreciated.

EXHIBIT C - REFERENCE QUESTIONNAIRE PACKAGE

EXHIBIT C PACKAGE INFORMATION AND FORMS - A “Final” Performance Evaluation (Construction) (also referred to as CCASS Evaluation) issued for a project may be included in lieu of sending out reference package for the project. Place the CCASS form directly behind the applicable Exhibit A project.

Instructions to Offeror for sending Reference Questionnaire Forms: Prepare and submit to the owners of the projects listed in your Exhibit A “Performance Relevancy Questionnaires”, a Reference Questionnaire Package (Exhibit C). For Government contracts, send to Contracting Officer or Technical Representative. For commercial references send to personnel with duties similar to those for Government contracts. Offerors should follow-up and encourage references to get the questionnaires to the Contracting Officer in a timely manner. If you have multiple references at one location (Base etc.), send one cover letter and questionnaire for each contract you want a reference for. Your questionnaire package should contain the following:

1. Cover Letter - Refer to Sample Transmittal Letter and Past Performance Evaluation Questionnaire (Exhibit C1)

2. Respondent Identification and Ratings (Exhibit C2) - Offeror should put name in spaces indicated as “…Contractor Submitting Proposal fill-in” and ensure your name is on every page for identification purposes.

Suggestion – Send an email to Contracting Officer or technical representative. Highly recommend including a Requested Delivery and/or Read Receipt to ensure it was received and read by the individual.
