SUMMARY OF CHANGES

SECTION M - EVALUATION FACTORS FOR AWARD

The following have been modified:

EVALUATION FACTORS FOR AWARD

1. BASIS FOR AWARD. Award will be made to the responsible Offeror offering the Lowest Priced, Technically Acceptable (LPTA) proposal, so long as the individual and overall prices proposed are reasonable. Each technical evaluation factor must be found to be “ACCEPTABLE” in order for the proposal to be found technically acceptable. Proposals will not be ranked based on technical merit; a given proposal is either "technically acceptable" or "not technically acceptable".

The successful Offeror’s pricing, Key Personnel, and any identified subcontractor(s) proposed will be incorporated into any resulting contract. The contractor must use the identified Key Personnel and subcontractor(s) in and throughout the performance of the contract work. If any of the Key Personnel or subcontractor(s) identified in the proposal are (or become) unavailable to perform the contract work, alternate Key Personnel or subcontractor(s) may be proposed by submitting the name and relevant experience to the Government in the form of a submittal prior to those individuals/entities performing work in that capacity. The qualifications and experience of the Key Personnel or subcontractor(s) proposed by the successful Offeror will be considered the minimum acceptable qualifications for any proposed replacements during contract performance. In the event that the contractor cannot provide one or more of the listed Key Personnel or subcontractor(s) (or one of comparable qualification in the sole discretion of the contracting officer), the contracting officer may reduce the contract price or take such other actions as are necessary to protect the interests of the Government (the interests of the Government including the timely performance and completion of the contract work).

It is the Offeror’s responsibility to ensure the completeness of its proposal. The Government may evaluate any given offer solely on the information provided in the initial proposal and will not assume that an Offeror possesses any capability that is not specifically identified in the proposal. In the event of doubt, ambiguity, or insufficient information to determine whether a given factor is technically acceptable, the factor will be deemed "not technically acceptable".

Failure to submit mandatory information for a factor, indicated by the word “shall” or similar verbiage in the solicitation, will result in a rating of “unacceptable” for that factor, and the offer will be ineligible for award unless the Contracting Officer decides to hold discussions and includes the offer in the competitive range.

2.SOLICITATION SUBMITTAL REQUIREMENTS. The proposal shall be submitted in two sections, a Technical Proposal and a Price Proposal. All pages containing text shall be consecutively numbered, type-written on 8 ½ x 11 inch paper with margins of at least one inch, and created using a font no smaller than 12 point. Alternate proposals will not be considered; each Offeror is limited to only one (1) proposal for this solicitation.

Technical Proposal: Offerors shall submit one (1) original and two (2) single-sided hard copies. The Technical Proposal shall include the following:

The Solicitation number (“W912ES-16-R-0005”);

The Offeror’s name, address, telephone number, and email address;

Names, titles, telephone numbers, and email addresses of persons authorized to negotiate with the Government on the Offeror’s behalf in connection with this solicitation;

Name, title, and signature of person authorized to sign the offer;

Documentation evidencing those individuals authorized to negotiate or execute offers have actual authority to bind the Offeror (appropriate to the partnership, company, corporation, or joint venture demonstrating the agency relationship (i.e. copies of corporate resolutions, powers of attorney, or official meeting notes));

Responses to all technical evaluation factors. Proposals must be organized along the subject matter areas (Parts I through III) described below. All information in the Offeror’s Technical Proposal must clearly indicate the Parts to which it is applicable:

o Part I: Key Personnel o Part II: Work Plan o Part III: Past Performance

Parts I and II of the Technical Proposal (Key Personnel and Work Plan) combined shall be no more than 30 pages (not including training certifications);

The Technical proposal shall not include pricing information.

Price Proposal: Offerors shall submit one (1) hardcopy single-sided original Price Proposal.

The Price Proposal shall include the following:

Completed Standard Form (SF) 33, including acknowledgement of Amendments;

Completed Pricing Schedule (Section B);

Proof of System for Award Management (SAM) registration along with any applicable representations and certifications (Section K).

3. EVALUATION FACTORS. The evaluation factors are listed below. The Government intends to evaluate proposals and make award without discussions with the Offerors (other than communications conducted for purpose of clarification[s]). If discussions are deemed necessary, discussions will occur with all Offerors unless the Contracting Officer establishes a competitive range; if a competitive range is established, discussions will be held only with Offerors in the competitive range. Failure to submit mandatory information for a factor, required by the word shall or similar language, will result in a rating of “unacceptable” for that factor and the offer will be ineligible for award unless the Contracting Officer decides to hold discussions, includes the Offeror in the competitive range, and the revised offer cures the deficiencies. Submission of requested information, indicated by the word should, is not mandatory; however, missing, incomplete, or ambiguous information may result in a rating of “unacceptable” for that factor.

The technical evaluation factors - Key Personnel, Work Plan, and Past Performance - will be evaluated first. Price will be evaluated after all technical evaluations are complete.

a. Key Personnel

The following positions are considered key personnel for purposes of this acquisition: Project Manager (PM), Alternate Project Manager (APM), Quality Control Manager (QCM), and Site Safety and Health Officer (SSHO). Key Personnel shall perform only the duties of the position to which he or she is assigned and shall be assigned to that position on a full-time basis during the period of performance of the contract. The only exception to the requirements of the previous sentence is that the same person may perform both the QCM and SSHO duties. If the Offeror chooses to combine the QCM and SSHO duties into a single position, the person identified for that position shall be assigned to that position on a full-time basis during the period of performance of contract. For each Key Personnel candidate proposed, the Offeror shall provide a resume and training certifications; the submitted information must clearly explain why the candidate proposed meets the minimum qualifications requirements in paragraphs (1) – (3) below. Offerors may submit only 1 candidate for each of the Key Personnel positions. Offers failing to identify all required Key Personnel candidates or identifying more than one candidate for any Key Personnel position will not be considered for award.

(1) Project Manager / Alternate Project Manager: The PM and APM shall each have a minimum 3 years of relevant experience managing facility maintenance projects similar to the scope of requirements included in this solicitation, e.g. road maintenance, snow plowing, and grass cutting.

(2) Quality Control Manager: The QCM shall have a minimum of 3 years experience in quality control inspection on service or construction projects, possess formal QC training, and shall have completed the 30 hour Occupational Safety and Health Act (OSHA) Construction Safety Class or equivalent, Competent Person Training, and Hazardous Waste Operations and Emergency Responders (HAZWOPER) training within the past 3 years.

(3) Site Safety and Health Officer: The SSHO shall have completed the 30 hour

OSHA Construction Safety Class or equivalent, Competent Person Training, and HAZWOPER Training within the past 3 years. The SSHO shall have five

(5) years of continuous safety experience as a minimum. The SSHO shall also provide other safety-related training that has been taken to maintain competency since completion of OSHA 30 hour training.

b. Work Plan

(1) Offerors shall submit a complete, detailed and feasible narrative description of their proposed Work Plan. The Plan shall address how the offeror will mobilize labor, materials, and equipment resources that will be used to complete the PWS (road maintenance, snowplowing, and grass cutting) in an efficient and timely manner.

(2) The number of crews, crew sizes, pieces of equipment, and equipment make / model shall be included as well as anticipated work schedules (days, hours / shifts) and anticipated daily production rates for grass cutting.

(3) Offerors shall also submit detailed information on the work being performed by the contractor and its subcontractors, and how the contractor is going to perform no less than 50% of the cost of contract performance incurred for personnel per contract clause FAR 52.219-14, Limitations on Subcontracting.

The Work Plan will receive a rating of "acceptable" or "unacceptable". In order to receive an "acceptable" rating, the proposed work plan shall contain: 1) all significant project activities; and 2) must demonstrate a clear understanding of the project activities in light of the PWS, site conditions, equipment proposed, and the environmental and other restrictions contained in the contract documents. Noncompliant, missing, incomplete, or ambiguous information may result in a rating of "unacceptable".

c. Past Performance – Offerors must submit completed Past Performance Questionnaires

(See Attachment 2) with their offer. Blocks 1-4 of the questionnaire must be completed by the Offeror and blocks 5-8 of the questionnaire must be completed by the Offeror’s reference. Offerors shall provide information for at least (2) projects for work completed by the Offeror within a five (5) year period preceding the date for initial receipt of proposals.

If less than two completed past performance questionnaires are submitted, the submitted project will be evaluated, and the lack of the requested number of projects will not affect the rating for this factor, provided the Offeror certifies that additional past performance information is not available and provides an acceptable explanation as to why additional past performance is not available.

In the case of an Offeror with no relevant performance history, or for whom information on past performance is not available, the Offeror will be evaluated as acceptable provided the Offeror certifies that additional past performance information is not available and provides an acceptable explanation as to why past performance information is not available. Absent that certification and explanation, an offer lacking some or all of the required past performance information will be deemed complete and receive a rating of “unacceptable” for Past Performance.

The past performance references must be from a client, or client representative in the case of a corporate client, with whom the Offeror had a contract, or subcontract (if applicable) for one or more projects. The reference must hold, or have held at the time of the past performance, a senior position in the client organization with responsibilities of contract administration, quality control, engineering, or technical supervision, or was otherwise in a position to evaluate the Offeror’s performance in executing the contract or subcontract.

The past performance reference must have first- hand knowledge of the Offeror’s performance. The reference must have a current, valid phone number at which it can be reached during normal business hours, an e-mail address or fax number, and must be able to speak, read, and write in the English language.

The Government’s assessment of past performance will be based primarily on the Offeror’s reputation with customers and other sources. The Government will consider the currency and relevance of past performance information, the source of the information, context of the data, and general trends in the contractor’s performance. The Government may contact any or all of the past performance references provided by the Offeror in response to this solicitation, and may also use information obtained from any other sources when evaluating past performance of the Offeror and subcontractors.

Other sources may include, but are not limited to, the Past Performance Information Retrieval System (PPIRS). While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance rests with the Offeror.

The evaluation will take into account relevant past performance information regarding predecessor companies or subcontractors that will perform major or critical aspects of the requirements when such information is available and relevant to the instant acquisition. If necessary during the evaluation process, any adverse information received regarding subcontractor past performance may be shared with Offerors during clarifications or discussions if held.

d. Price

(1) Offerors shall complete and submit the price schedule with their proposal and include a price for each line item. Failure to do so will result in the proposal being rejected without further consideration.

(2) The SSEB will evaluate the lowest priced technically acceptable Offeror’s price in accordance with FAR 15.4 to determine reasonableness in comparison to the IGE and the work to be performed. In this context, reasonableness will be determined by comparing offered prices to the government estimate.
