1.1 REFERENCES

1.2 SUBMITTALS
Government approval is required for submittals with a "G" designation;
submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. Submittals with an "S" are for inclusion in the Sustainability Notebook, in conformance with Section 01 33 29 SUSTAINABILITY REPORTING. Submit the following in accordance with Section 01 33 00 SUBMITTAL PROCEDURES:

1.3 COORDINATION AND ACCESS TO SITE
1.3.1 Coordination with Using Agency
Coordination with using agencies shall be made through the Contracting Officer to assist the Contractor in completing the work with a minimum of interference and inconvenience.
1.3.2 Work hours
Work hours in the construction area will be restricted to 7:00 a.m. to 5:00 p.m. Monday through Friday, excluding government holidays. Work hours
SECTION 01 14 00 Page 2 other than as specified above may be approved by the Contracting Officer and will be reviewed on a case by case basis. Provide any request, for work hours other than as specified above, at least 21 days in advance.
1.4 FIRE REGULATIONS
Comply with base fire regulations and NFPA 241 Building Construction and Demolition requirements. Provide adequate fire extinguishers for the construction site and remove them upon acceptance of the facility.
1.5 GENERAL AREA REQUIREMENTS
Activities of the Contractor and Contractor's employees and subcontractors and their employees while on the base, will be conducted in accordance with base regulations, including those of the fire marshal, as well as security directives. This includes, but is not limited to, obtaining a Base Civil Engineer Work Clearance Request (AF Form 103, Attachment 01 14 00-A included at the end of this section) before any digging and yielding to alert vehicles during alerts if located on a marked alert route. Security directives include requirements from Paragraph ANTITERRORISM FORCE PROTECTION below, and the Contractor Entry Authorization List (Attachment 01 14 00-B included at the end of this section). This list shall include all Contractor personnel working on the base.
1.5.1 Base Civil Engineering Work Clearance Request
Prior to the start of field operations and prior to any excavation performed under the subject contract, an approved Base Civil Engineering Work Clearance Request (AF Form 103 included as Attachment 01 14 10-A) commonly known as a "digging permit" must be in the possession of the Contractor. The document shall be fully executed and signed by an approving officer of the Air Force. The permit will take a minimum of 21 calendar days to procure after a written request is received from the Contractor. The Contractor shall be solely responsible for all repairs and damages occurring as a result of construction operations performed without a properly execute AF 103 and proper locates. The initial approved Civil Engineering Work Clearance Request is valid for 90 days and a new request shall be made every 90 days as required.
It is the sole responsibility of the Contractor to make all requests for locates and marking to Air Force shops, groups, contractors, or other parties listed on the permit (AF Form 103). It is also the sole responsibility of the Contractor to make all requests for locates and marking by the private sector. Note that private companies supply numerous utilities and services on Malmstrom Air Force Base. AF Form 103 does not address coordination with private companies. Requests for locates by private firms shall be performed by the Contractor in accordance with State of Montana and Federal regulations. Once the Air Force has located utilities on-site, it is the responsibility of the Contractor to preserve those markings. The Contractor should survey in utility marking locations.
Relocation will only be done on a case-by-case basis.
1.5.2 Contractor Employee Access to Malmstrom AFB
1.5.2.1 General
All Contractor personnel requiring access to Malmstrom AFB will be required to process in through the prime Contractor on an approved Contractor Entry Authorization List (CEAL) (Attachment 01 14 10-B included at the end of
SECTION 01 14 00 Page 3 this section). The Malmstrom Resident Office will provide an electronic copy of this document upon request.
The prime Contractor shall forward via email a completed copy of the CEAL to the designated COE representative at the Malmstrom Resident Office. A background check will be done on ALL individuals seeking entry onto Malmstrom AFB. This will take approximately 5 business days.
Information required is full name (first, MI, last), SSN, DOB, State Issued Driver's License number (e.g. MT 1234567890000) or REAL ID Act compliant ID (e.g. passport number), height /weight, eye color/hair color, sex, current residence address, company and type of access required. The personnel on the CEAL shall be arranged alphabetically by last name.
Once the pre-screened individual(s) have been cleared they will then proceed to Building #192 (6220 Goddard Drive, located right outside the 2nd Avenue North Main Gate), obtain a visitor's pass (AF Form 75), be waved on to the base, then proceed to Building #250 (7532 Flightline Drive) Room 1.
They must provide a state/government issued picture identification to be verified against the CEAL.
1.5.2.1.1 Identification Credentials
The REAL ID Act of 2005 established minimum standards for the production and issuance of state-issued driver's licenses and ID cards which include requirements for a photograph and certain biographic information, such as name, date of birth, gender, height, eye color, & address. State-issued driver's licenses and ID cards from states not meeting the standards can no longer be used for accessing Federal facilities including Air Force installations unless the issuing state's compliance deadline has been extended by the Department of Homeland Security (DHS). Individuals, who do not possess a driver's license or identification card compliant with the REAL ID Act and do not possess an acceptable alternate identification credential, must be escorted or denied access. Refer to the Department of Homeland Security website for the most current status, https://www.dhs.gov/current-status-states-territories.
State-issued driver's licenses and ID cards from states not meeting the 2005 REAL ID Act standards cannot be used for accessing federal facilities including Air Force installations unless the issuing state's compliance deadline has been extended by the Department of Homeland Security (DHS).
Individuals, who do not possess a driver's license or identification card compliant with the REAL ID Act and do not possess an acceptable alternate identification credential, must be escorted or denied access.
An identification card issued by a Government agency may be used if the individual does not possess a driver's license and the identification number provided must be verifiable (i.e. Passport Number). Take special care to ensure all information on the CEAL for each has been verified to be accurate prior to submitting for approval. The Government will not be responsible for delays caused by disapproval of Contractor access requests due to inaccurate information provided by the Contractor.
1.5.2.1.2 Retired Military ID
Retired military members and dependents of active duty or retired military members who are Contractor employees may use their DoD identification when entering MAFB for the performance of contract services. Under no circumstance will retired military members or dependents of active duty or
SECTION 01 14 00 Page 4 retired military members vouch Contractor personnel onto the installation utilizing their DoD identification card. All DoD contractors who have been issued a Common Access Card will be allowed to sign people onto the installation.
1.5.2.1.3 Procedures for Base Access
All local contractors or vendors must comply with the following:
Upon award of a contract, the Contractor will be issued a request for a Contractor Entry Authority List (CEAL) by the contract administrator. Upon receipt, compile the information required for the CEAL and submit a completed CEAL to the contract administrator.
The contract administrator will approve the CEAL and provide it to 341 SFS/S5B for review. 341 SFS/S5B personnel will compare the approving official's signature against an AFGSC Form 577, or an appropriate letter on file at pass and registration.
The Contractor names will be vetted via CJIN for wants and warrants.
Unfavorable results will be marked UNF, lined through, and result in individuals being denied access to the installation. 341 SFS will have 5 duty days in which to complete background checks on US citizens and 10 days for foreign nationals. Security managers will utilize this information when completing EALs for entry to restricted, limited, and controlled areas as proof of a CJIN check. This check must be completed annually on each contractor/vendor.
Contractor personnel requesting a contractor pass will provide a REAL ID Act compliant ID card to be verified against the CEAL. The CEAL will need to be resubmitted in its entirety any time personnel need to be added. New personnel shall be highlighted in yellow. Only one update to the CEAL will be allowed per week. Advance planning of personnel working on the contract will be required to meet this requirement.
Those employees who clear the background check process will be issued passes/DBIDS cards by 341 SFS/S5B for the duration of the contract or one year, whichever period is shorter. For contracts lasting more than one year -- including contract options, renewals, and extensions -- the Contractor must repeat the above stated process on an annual basis. In addition, the process must be initiated for all new employees who are added to the CEAL after its initial submission. Personnel who possess a DoD ID card will not be issued a pass/DBIDS card.
1.5.2.1.4 Causes for Denial of Access
The 341 SFS/CC will deny the individual access onto Malmstrom for the following fitness determinations or similar offenses:
a. The individual is known to be or reasonable suspected of being a terrorist or belongs to an organization with known terrorism links/support.
b. The individual's claimed identity cannot be verified.
c. There is a reasonable basis to believe the individual has submitted fraudulent information concerning his or her identity. NOTE: If Security Forces suspects an individual is attempting to provide fraudulent documents for the purpose of defeating identity proofing
SECTION 01 14 00 Page 5 procedures, the individual will be detained and Security Forces Investigations and AFOSI will be notified.
d. There is a reasonable basis to believe the individual will attempt to gain unauthorized access to classified documents, information protected by Privacy Act, information that is proprietary in nature, or other sensitive or protected information.
e. There is a reasonable basis to believe the individual will unlawfully or inappropriately use an access credential outside the workplace.
f. There is a reasonable basis to believe, based on an individual's criminal history, that issuance of an access credential poses an unacceptable risk.
g. The individual has/had been debarred from entry/access to a Federal installation or facility. NOTE: An individual who has/had been debarred from another Federal Installation must receive approval from the 341 MW/CC prior to entry to MAFB utilizing the appeal process listed in Paragraph APPEALS PROCESS below.
h. The individual is wanted by Federal or civil law enforcement authorities, regardless of offense or violation.
i. The individual has been incarcerated for 12 months or longer within the past 10 years, regardless of offense/violation, unless released on proof of innocence.
j. The individual has any convictions for espionage, sabotage, treason, terrorism, or murder.
k. The individual's name appears on any Federal agency's "watch list" or "hit list" for criminal behavior or terrorist activity.
l. The individual has been convicted of a firearms or explosive violation within the past 10 years.
m. The individual has been convicted of sexual assault, armed robbery, rape, child molestation, child pornography, trafficking in humans, drug possession with intent to sell or drug distribution.
n. There is a reasonable basis to believe, based on the individual's material, intentional false statement, deception, or fraud in connection with Federal or contract employment, that issuance of an access credential poses an unacceptable risk.
o. There is a reasonable basis to believe, based on the nature or duration of the individual's alcohol abuse without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk.
p. There is a reasonable basis to believe, based on the nature or duration of the individual's illegal use of narcotics, drugs, or other controlled substances without evidence of substantial rehabilitation, that issuance of an access credential poses an unacceptable risk.
q. A statutory or regulatory bar prevents the individual's contract employment; or would prevent Federal employment under circumstances that furnish a reasonable basis to believe that issuance of an access

r. The individual knowingly and willfully engaged in acts or activities designed to overthrow the U.S. Government by force.
s. Extortion.
t. Kidnapping or hostage taking.
u. Any affiliation, membership, or connection to a gang known for their criminal or terrorist associations.
Any felony conviction involving:
v. Sedition (Conspiracy).
w. Arson.
x. Registered Sex Offenders.
y. Violence at international airports (Title 18 USC 37).
z. Conspiracy or attempt to commit any of the criminal acts in the preceding paragraphs.
The individual has been convicted of a felony for any of the following within the last 10 years:
aa. Illegal possession of a controlled substance.
bb. Acts involving a threat.
cc. Willful destruction of property.
dd. Importation or manufacture of a controlled substance.
ee. Burglary, theft, or larceny.
ff. Dishonesty, fraud, or misrepresentation.
gg. Possession or distribution of stolen property.
hh. Aggravated assault.
ii. Bribery.
1.5.2.1.5 Appeal Process
Requestors may appeal the initial decision to deny installation access in writing to 341 SFS/S5, 21 7506 Flightline Drive, Malmstrom AFB, MT 59402-7538. Provide a copy of the appeal to the Contracting Officer.
Appeals should include any available court-related documentation or other documentation necessary to disprove the disqualifying information. Upon receipt of the written information, the 341 SFS/CC will reassess whether the individuals request for installation access should be granted.
In the event the 341 SFS/CC does not recommend access based on the written information provided, the package will be sent to the 341 MSG/CC for a final determination. In cases where an individual has/had been debarred
SECTION 01 14 00 Page 7 from entry/access to another installation, the package will be sent to the 341 MW/CC for a final determination.
1.5.2.1.6 Replacement of Lost/Stolen DoD ID Card
Personnel must request the replacement of a DoD ID card in accordance with AFI 31-113, paragraph 1.5.17.4.-1.5.17.4.2. The MPS Customer Support Office will provide an ID Card Reissue Letter to personnel that have lost their ID card or had their ID card stolen. Personnel must report the loss or theft of their ID card to the 341 SFS/S5 (Bldg 295). 341 SFS/S5 will verify the ID card has not been turned into the security forces and if the ID card has been lost or stolen deregister the ID card in DBIDS. The 341 SFS/S5 will sign an ID Card Reissue Letter and then personnel will contact their squadron commander or first sergeant (military personnel and their dependents), or Verifying Official (civilians and contractors) to verify the circumstances of the lost or stolen government property. Personnel will then take the completed ID Card Reissue Letter to the MPS Customer Support office to have a new ID card reissued.
1.5.2.1.7 Employee Termination
Contractors are required to notify the Malmstrom Resident Office, within 48 hours, if an employee on a CEAL terminates his or her employment with the Contractor or is involved in an incident resulting in an unfavorable result, including, but not limited to, those identified above.
If a Contractor terminates an employee, it is incumbent upon the Contractor to ensure that the employee's access badge is returned to the Malmstrom Resident Office. If they are unable to retrieve the pass, they will contact the Malmstrom Resident Office with the employee's information. The Malmstrom Resident Office will then inform Pass and Registration who will then contact the installation entry control points and have them retrieve the pass if the person attempts to re-enter the base.
If a contractor has a DBIDS issued card, it will be invalid for installation entry immediately upon notification to Pass and Registration.
The card must still be returned to the Malmstrom Resident Office for destruction. The passes shall not be worn or displayed off the military base.
1.5.3 Commercial Vehicle Access Restrictions
Commercial or company vehicles will be allowed access to the base provided company emblems are attached to the sides of the vehicles.
Commercial, delivery, and/or contract vehicles entering Malmstrom AFB must enter through the 10th Avenue North gate. The 10th Avenue North gate is open for commercial vehicles from 6:00am to 6:00pm Monday through Friday and closed on weekends and holidays. Provide 21 day advance notice when commercial vehicles will be required to enter Malmstrom AFB on Saturdays when work is allowed (see paragraph WORK HOURS) so arrangements can be made for the Government to open the 10th Avenue North Gate.
Commercial, delivery, and/or contract vehicles entering MAFB will be searched for explosives. Installation Entry Controllers have the authority to direct a search of any vehicle for explosives based on current threat and vehicle status. The following vehicle types will be searched:
(1) Commercial vehicle: any vehicle that is registered to a company rather

(2) Delivery vehicle: any vehicle making a delivery to MAFB.
(3) Contractor vehicle: any truck, tractor, trailer that a contractor drives or tows on base.
1.5.4 Equipment and Storage Areas
See specification Section 01 50 00 TEMPORARY CONSTRUCTION FACILITIES AND
CONTROLS.
1.5.5 Antiterrorism Force Protection
During the course of this contract, the Base may be under Force Protection Conditions (FPCONS). FPCONS are as follows:
NORMAL: This condition applies when there is a general threat of possible terrorist activity exists warrants only a routine security posture.
ALPHA: This condition applies when there is a general threat of possible terrorist activity against personnel and facilities, the nature and extent of which are unpredictable, and circumstances do not justify full implementation of FPCON BRAVO measures. However, it may be necessary to implement certain measures from higher FPCONS resulting from intelligence received or as a deterrent. The measures in this FPCON must be capable of being maintained indefinitely.
BRAVO: This condition applies when an increased and more predictable threat of terrorist activity exists. The measures in this FPCON must be capable of being maintained for weeks without causing undue hardship, affecting operational capability, and aggravating relations with local authorities.
CHARLIE: This condition applies when an incident occurs or intelligence is received indicating some form of terrorist action against personnel and facilities is imminent. Implementation of measures in this FPCON for more than a short period probably create hardship and affect the peacetime activities of the unit and its personnel.
DELTA: This condition applies in the immediate area where a terrorist attack has occurred or when intelligence has been received that terrorist action against a specific location or person is likely.
Normally, this FPCON is declared as a localized condition.
If the Contractor should notice anything suspicious during the course of his work, the Contractor should contact the 911 Dispatch Center at 406-731-3895 as soon as possible. Point of Contact for FPCON conditions/actions is 341st Missile Wing AT/FP Section; phone 406-731-4105.
1.6 CONSTRUCTION AND STAGING AREA FENCE
Provide fence around all construction areas, staging areas and storage areas. The fence shall be chain link, 6 feet high, with construction area signage at each corner and at 100-foot intervals.

1.7 MOTORIZED EQUIPMENT
1.7.1 Truck Load Limits
Truck load limits on base are restricted to:
a. April - 1 June: 350 pounds per inch width of tire.
b. All other times: 400 pounds per inch width of tire.
Trucks shall periodically be required to pass over the base truck scale to verify compliance with the above vehicle load limits.
1.7.2 Fire Extinguishers
Motorized equipment shall be equipped with fire extinguishers as follows:
a. Pickup truck or other light passenger vehicles, one extinguisher per vehicle, rating 5 BC.
b. All other trucks and heavy motorized equipment, two extinguishers per vehicle, rating 10 BC.
1.8 UTILITY OUTAGES
Schedule and coordinate unavoidable utility outages with the Contracting Officer and Base Civil Engineer at least 21 days in advance. Unless indicated otherwise, give 21 days preliminary notice of future outage.
Final notice shall be given 21 calendar days before outage and shall specify date and time of the outage. The 'Utility Outage Notice' shall be completed by the Contractor and submitted to the Contracting Officer for approval. No interruptions shall be made until the approved Notice is returned to the Contractor. Outage durations shall be kept to a minimum.
All outages longer than 2 hours shall be after normal duty hours or on a weekend at the Government option. Outages in dormitories or housing units shall be performed during business hours to minimize disruption to occupants. All outages that affect heating system motors or controls during heating season shall require the Contractor to connect backup power at the Contractor's expense during the outage, as may be required by the Government.
1.9 UTILITY SERVICE AND COORDINATION
1.9.1 Natural Gas Service
Extension of and/or connection to the existing natural gas distribution system is the responsibility of the Contractor. All natural gas work shall be performed in accordance with the supplier's requirements. Additionally, all natural gas work must be performed as depicted on the drawings, and as required by the contract specifications.
1.9.2 Electrical Service
All electrical distribution work including the street light system and temporary and permanent electrical services shall be performed by Contractor. All electrical distribution work shall be performed in accordance with the supplier's requirements and the most current versions of the National Electric Safety Code and the National Electric Code (NFPA 70 ). Additionally, all permanent electrical distribution work must be
SECTION 01 14 00 Page 10 performed as depicted on the drawings, and as required by the contract specifications. In any case of conflict between the applicable codes or references listed above, the most restrictive shall apply.
1.9.3 Telephone Service
Telephone cable and pedestal installation will be by Contractor. Network interface devices (NID) and installation will be by Contractor. Site trenching, backfill and the installation of conduit shall be performed by Contractor.
1.9.4 CATV Service
CATV cable and pedestal installation will be by Bresnan Communications.
Network interface devices (NID) and installation will be by Bresnan Communications. Site trenching, backfill and the installation of conduit shall be by Contractor. Point(s) of Contact for Bresnan Communications will be provided by Base.
1.10 PROTECTION OF GOVERNMENT PROPERTY
In addition to requirements of the CONTRACT CLAUSES, protect all Government property within the buildings in which he is working, except for such property as is required to be demolished. Property that is to be demolished shall be protected until its scheduled demolition time.
Protection shall include, but not be limited to, protection from construction generated dust, debris, water, and vibration.
1.11 COORDINATION OF WORK
Arrange work schedule to maintain two-lane road passage at all times unless prior approval is granted by the Contracting Officer. The Contractor shall be responsible for coordinating lane closures with Malmstrom Air Force Base emergency response personnel (Base Fire Department, Law Enforcement, and ambulance services), city and county emergency response personnel, trash collection contractor and school bus operators. A Work Schedule shall be prepared in writing and approved by the Contracting Officer least 14 days prior to the start of work. A Traffic Control Plan prepared in accordance with the most recent version of the Manual on Uniform Traffic Control Devices shall be submitted and approved by the Government prior to any road demolition, construction, or lane closure.

STATEMENT PURPOSE
This Operating Instruction (OI) outlines the procedures and requirements for AF Form
103, Base Civil Engineer Work Clearance Requests, as it applies to all units, individuals and contractors assigned, attached, or working on Malmstrom AFB, and it’s Geographically Separated Areas (GSAs).
SUMMARY OF REVISIONS
The current AF Form 103 with updated contact information is attached for reference.
This Operating Instruction clarifies the responsibilities for Base Civil Engineers, civilian contractors, and other agencies working with, for and on Malmstrom AFB to include the Malmstrom AFB Missile Complex while processing AF Form 103.

1. Reference. Montana Utilities Coordinating Council (MUCC); Montana Code Annotated
(MCA) 69-4-501; Air Force Form 103, Base Civil Engineer Work Clearance Request.
2. General. This OI is designed for utility companies, governmental agencies, contractors, and other interested organizations or individuals to reduce damages to subsurface installations and as-well-as above.
3. Definition of Terms.
3.1. AF Form 103. Otherwise known as Base Civil Engineer (BCE) Work Clearance
Request, or digging permit.
3.2. Permit requester. Any Civil Engineer, civilian contractor, or other agency that is requesting permission to dig on Malmstrom AFB or its GSAs.
3.3. Approximate location of subsurface installation. A strip of land not more than 36 inches on either side of the exterior surface of the subsurface. Does not mean depth.
3.4. Duty day. A day other than Saturday, Sunday, or a legal local, state, or federal holiday to include AF Global Strike Command Family Days.
3.5. Utility. Any electric, gas, water, sewer, storm drain, petroleum, telephone, fiber, high temp hot water (HTHW) and/or television installation. Term may also apply to the agency or organization owning or controlling the service.
3.6. Excavation. Any operation in which earth, rock, or other materials in the ground are moved, removed, or otherwise displaced at least four (4) inches, in depth, by means or use of any tools, equipment or explosives. The term excavation includes, but not limited to, grading, trenching, digging, ditching, auguring, tunneling, scraping, cable or pipe plowing, boring or driving, flow line, surface road or ditch grading, to include surface or ditch maintenance.
3.7. Identify buried un-locatable underground facility. An underground facility that can be field- marked with reasonable accuracy.
3.8. Mark. The use of stakes, chalk, flags, or other clearly identifiable materials to show location, or absence, of underground facilities in accordance with the current color-code standard of the American Public Work Association. Marking must include identification letters indicating the specific type of underground utility and the width of the utility.
3.9. Reasonable accurate. A location within 36 inches of the outside lateral dimensions of both sides of an underground facility or utility.

3.10. Underground utility. Any Utility buried or placed below ground for use in connection with the storage or transportation of water, sewage, electronic telephone or communications, cable television, fiber optics, electrical energy, HTHW, oil, gas, steam or their substances. The term includes but is not limited to; pipes, sewers, conduits, cables, valves, lines, wires, storm water, manholes and attachments to the listed items.
3.11. Permit issuer. The Civil Engineer Squadron’s Customer Service (CE Customer Service) and Missile Engineering are the sole entities that will issue the AF Form 1 0 3 .
3.12. Subsurface installation. Includes, but not limited to; any underground pipe, sewer, conduit, cable, valve, line or wire.
4. Base Side Procedures.
4.1. A work request must first be approved prior to the routing of the AF Form 103. The project number or the work order number on the AF Form 332 or Direct Schedule Work Order will be used as a tracker for the AF Form 103 as it is being routed for coordination.
4.2. The permit requester will mark the excavation site with white chalk, flags or stakes.
4.3. The permit requester will visit the Civil Engineering Drafting and Surveying section
(731-6633) and receive/bring, at a minimum, five copies of the requested dig area map to CE
Customer Service to create an AF Form 103.
4.4. Once issued the AF Form 103, the requesting official or the appointed designee shall walk the permit through all of the required agencies annotated on the form to include local civilian entities.
4.5. After every required agency has seen, marked, staked or flagged, and approved the location of the dig; the requesting official will bring the AF Form 103 into CE Customer
Service.
4.6. A CE Customer Service representative will determine the approval authority and will take the dig permit to one of the following for final signature and approval: The Base Civil
Engineer or Deputy, the Chief of Operations or Deputy, or Chief of Engineering or Deputy.
4.7. CE Customer Service will furnish the permit requester with a copy of the approved AF
Form 103.
4.8. The approved AF Form 103 will be valid for a period to not exceed 90 calendar days as written in block 4 of the AF FORM 103. Renewal can be accomplished; however, this requires the signature of the approval authority for one additional 30 calendar day period.
Renewal signature will be completed prior to the last day of the initial 90 day cycle, and utility

16031 locates will be re-accomplished if delays or work stoppages exceed 90 days (excluding winter exclusion periods). Approval to hold an approved AF Form 103 due to winter exclusion periods are required to be approved by the Base Civil Engineer or Deputy, the Chief of
Operations or Deputy, or Chief of Engineering or Deputy.
4.9. A copy of the approved AF Form 103 will be kept at the job site in the foreman’s possession. The work requester, contractor, and/or excavators shall hand dig or hydro-vac around the approximate location of subsurface installations or utility.
4.10. If required, prior to back filling the location of the dig, the entity accomplishing the job or project must notify Civil Engineering Drafting and Surveying at 731-6633 to annotate depth and size of utility line.
4.11. Once the excavation is completed the permit requester will notify CE Customer Service.
4.12. After CE Customer Service makes note of the completion of the dig and contacts the
Drafting and Surveying Section to make sure the line has been annotated in the Geobase program, the backfilling process may begin.
5. Missile Field Procedures.
5.1. A work request must first be approved prior to the routing of the AF Form 103. The project number or the work order number on the AF Form 332 or Direct Schedule Work Order will be used as a tracker for the AF Form 103 as it is being routed for coordination.
5.2. The permit requester will mark the excavation site with white chalk, flags or stakes.

The requestor shall call MXG Plans and Scheduling at 731-3068 to schedule missile E comm to locate effected communications systems. The requestor shall call 811 for any work at the MAFs and any work outside the LF security fence.
5.5. After every required agency has seen, marked, staked or flagged, and approved the location of the dig; the requesting official will bring the AF Form 103 back to Missile
Engineering.
5.6. A Missile Engineering representative will determine the approval authority and will take the dig permit to one of the following for final signature and approval: The Base Civil

Engineer or Deputy, or Chief of Missile Engineering or Deputy.
5.7. Missile Engineering will furnish the permit requester with a copy of the approved AF
Form 103.
5.8. The approved AF Form 103 will be valid for a period not to exceed the amended date in box 4 approved by the Base Civil Engineer or Deputy, or Chief of Missile Engineering or
Deputy.
5.9. A copy of the approved AF Form 103 will be kept at the job site in the foreman’s possession. The work requester, contractor, and/or excavators shall hand dig or hydro-vac around the approximate location of subsurface installations or utility.
5.10. If required, prior to back filling the location of the dig, the entity accomplishing the job or project must notify the Missile Engineering Drafting section at 731-7531 to annotate depth and size of utility line.
5.11. Once the excavation is completed the permit requester will notify Missile Engineering.
5.12. After Missile Engineering makes note of the completion of the dig and contacts the
Drafting and Surveying Section to make sure the line has been annotated in the Geobase program, the backfilling process may begin.
6. Standard Color Code for Utility Locations
6.1. Active Utilities. Each organization or utility that has subsurface installation on
Malmstrom AFB, shall mark those utilities using the following color code:
6.1.1. RED. Electric power, distribution and transmission installation.
6.1.2. YELLOW. Gas and oil distribution and transmission installations or installations containing or transporting dangerous materials, products or stream.
6.1.3. ORANGE. Telephone or fiber, cable television, police and fire communication installation.
6.1.4. BLUE. Waterlines, HTHW, and slurry pipelines.
5.1.5. GREEN. Sanitary and storm sewers, and storm drains.
5.1.6. WHITE. Excavation area.
6.2. Abandoned Utilities. Shall be marked, when accessible, using the following color code and ABD symbol.

6.2.1. RED. Electric power, distribution and transmission installations chalk a triangle symbol with the capital letters “ABD” inside the triangle.
6.2.2. YELLOW. Gas and oil distribution and transmission installations or installations containing or transporting dangerous materials, products or stream. Chalk a triangle symbol with the capital letters “ABD” inside the triangle.
6.2.3. ORANGE. Telephone and fiber systems, cable television, police and fire communication installations. Chalk a triangle symbol with the capital letters “ABD” inside of the triangle
6.2.4.BLUE. Water lines, storm sewer and storm drain lines. Chalk a triangle symbol with the capital letters “ABD” inside the triangle
6.2.5. GREEN. Sanitary and storm sewers, and storm drains.
7. Marking Utilities
7.1. Stakes, flags or chalk must be used to accomplish utility markings. There shall not be any markings to the front face of curbs, gutters, sidewalks, pavement parking lots or roadways.
Color-coded markings can be placed on the top backside of curbs and shall be limited to a dot no larger than 1 1/2 inches in diameter. EXCEPTIONS: A parking lot, asphalt roadways, sections of sidewalk, curbs and gutters that have been scheduled and coordinated to be removed during construction, demolition, general maintenance, and/or safety trepidation can be chalked for removal.
7.2. Chalk, stake or flag a one (1) foot line every four (4) steps in the running direction of the buried utility.
7.3. Place flags in tall grass or bushy areas.
7.4. Use rooster (colored tops) and nails in gravel roadways.
7.5. Mark an 18-inch to three (3) foot circle around phone pedestals, electrical transformers and section cans.
7.6. All markings of utilities, water, steam, slurry, storm drains, natural gas, oil, electrical, telephone, communications, fiber, television, etc. shall be marked using COLOR-CODED chalk. No paint products shall be used on Malmstrom AFB or its GSAs.
8. Other
8.1. Once all utilities are marked and the AF Form 103 is approved, the contractor/user becomes responsible for keeping all marks, even after construction or demolition starts. This

16031 includes the removal of any stakes or flags used once the job is completed. Customer Service will keep a copy of the approved AF Form 103 for record and will also be the focal point for contacting the facility managers of buildings that may be affected during the excavation.
8.2. During excavations, if any subsurface installation or utility is found that was not originally marked, the contractor/user must contact the project manager, or inspector, from
Civil Engineering. Once the project manager/inspector determines who it belongs to, they will call the appropriate shops.
