About this file
This is a Technical Exhibit 2 (TE 2) template for Work Order Request/Repair and Cost Estimate submissions under a firm-fixed-price contract for HVAC repair services at Kahuku Training Area (KTA). The document establishes the format and requirements for contractors to submit repair quotes for equipment failures, with labor quoted as a firm-fixed-price lump sum rather than hourly rates. Contractors must include location/installation details, problem description, scope of work, itemized costs (labor, materials/parts, shipping/handling, tax, subcontractor services, and other charges), and estimated completion time or lead time if parts are unavailable. PM-identified repairs (PWS 8.8) require submission within two business days, while unscheduled repairs (PWS 8.11) require acknowledgment within one business day and submission within five business days of on-site assessment. The document includes two examples showing typical repair scenarios with cost breakdowns totaling $705.00 and $2,325.00 respectively.
Key contractual provisions include that all quotes constitute firm-fixed-price, all-in proposals with labor provided as a complete lump sum for the full repair scope. Contractors are prohibited from beginning repair work or ordering parts until the Contracting Officer issues a written FFP task order. All parts and materials must be new OEM-equivalent unless refurbished items are unavailable, and all components carry a one-year warranty. Any costs exceeding the approved FFP quote are the contractor's responsibility. Purchase receipts must accompany quotes for price analysis, though they do not serve as invoicing basis. The template designates yellow cells for contractor inputs and blue cells for calculations, excluding the example rows from government evaluation totals.
