About this file
This file is a Past Performance Questionnaire template used to evaluate contractor performance. The questionnaire assesses five key performance areas: Quality of Product/Service, Invoices, Schedule, Business Relationships, and Key Personnel. Each section contains multiple evaluation criteria that are rated as either "Acceptable" or "Unacceptable."
The template is structured to gather feedback on contractor performance including quality of deliverables, invoice accuracy and timeliness, schedule adherence, management effectiveness, communication, responsiveness to technical direction, key personnel turnover, and overall satisfaction. It requires evaluators to provide narrative explanations for any "Unacceptable" ratings. The questionnaire appears to be related to a Between Occupancy Maintenance (BOM) contract for Military Family Housing in Wiesbaden, Germany, though the template itself is a standard evaluation form that could be used for various contracts.
