About this file
This is a Quality Assurance Nonconformance Report (NCR) template used by the 409th Contracting Support Brigade (CSB) to document and track contractor performance issues. The form defines three levels of nonconformance: Level I (minor issues unlikely to affect usability), Level II (major issues likely to cause failure or reduced usability), and Level III (critical issues that could create hazardous conditions or prevent mission performance).
The template includes sections for documenting the COR and contractor information, description of noncompliance, contractor required actions (for Level II and III only), and 409th CSB final actions. For Level II and III nonconformances, contractors must provide six specific responses including root cause analysis, corrective actions, preventive measures, impact assessment on other deliverables, process improvements, and implementation timeline. The form requires signatures from both the contractor and Contracting Officer (KO) and allows 10 days for contractor response. This template is used across multiple Regional Contracting Offices including Bavaria, Benelux, Stuttgart, Rhineland Pfalz, and Wiesbaden.
