About this file

This document is a Small Business Commitment Document template for a federal solicitation. It requires offerors other than small businesses to provide details of their proposed commitments to small business subcontracting goals. Offerors must specify the percentages of work to be performed by large businesses, small businesses, and subcategories including small disadvantaged businesses, woman-owned small businesses, historically underutilized business zone businesses, veteran-owned small businesses, and service-disabled veteran-owned small businesses. They must also identify the prime contractor and intended small business subcontractors, including company names, unique entity identifiers or commercial and government entity codes, and anticipated North American Industry Classification System codes for each subcontract. Offerors are further required to describe the types and durations of commitments to small business subcontracting and provide copies of any written commitment agreements. Past performance information on meeting small business goals must be included for any projects submitted under Factor 1 of the solicitation.

The related federal contract opportunity is for the Bonneville Powerhouse 1 Main Unit Breaker Replacement solicited by the Department of the Army Corps of Engineers Engineering District Portland. No other details are provided about the required products or services.

ATTACHMENT C - SMALL BUSINESS COMMITMENT DOCUMENT

Other than small businesses (SB) Offerors are required to complete a Small Business Commitment Document (SBCD).

The Offeror shall propose the level of commitment of SB concerns (e.g., Small Disadvantaged Business (SDB) (including ANCs and Indian Tribes), Woman-Owned Small Business (WOSB), Historically Underutilized Business Zone (HUBZone), Veteran-Owned Small Business (VOSB), and Service-Disabled Veteran-Owned Small Business (SDVOSB)) in the performance of the acquisition relative to the objectives/goals set-forth in the evaluation of this factor.

A suggested Total Small Business Commitment of 5% of the total contract value is assigned to this acquisition for Small Business, as well as a suggested minimum 1% of the total contract value for all other socioeconomic categories, SDB, WOSB, VOSB, SDVO and HubZ.

1. Indicate the percentage of work based on total contract dollars of the acquisition to be performed by both large and SB (include the percentage of work to be performed by both the prime and both large and small business subcontractors):

For example: If the Prime proposes a price of $1,000,000 (including all options), and SB will provide $250,000 in supplies/services as a prime or subcontractor, the %-age planed for SB is 25%; and 75% for large business equating to 100%.

Total percentage planned for Large Business(es) = %

Total percentage planned to be self-performed by Prime = %

Total percentage planned for Small Business(es) = %

Total: 100%

2. Indicate the total combined percentage of work based on total contract dollars of the acquisition to be performed by each SB socioeconomic category. The work performed by any SB that qualifies in multiple SB categories should be counted in each applicable category.

For example: Subcontractor A (a VOSB, SDVOSB, and WOSB) performing 2%; and Subcontractor B (a SDB, HUBZone, and WOSB) performing 3%. The results will equate to SB 5%, SDB 3%, HUBZone 3%, WOSB 5%, SDVOSB 2%, and VOSB 2%).

Small Disadvantaged Business (SDB) (1% suggested goal) % Woman-Owned Small Business (WOSB) (1% suggested goal) % Historically Underutilized Business Zone (HUBZone) 1% Suggested goal) % Veteran-Owned Small Business (VOSB) (1% suggested goal) % Service-Disabled Veteran-Owned SB (SDVOSB) (1% suggested goal) %

3. Identify the Prime Offeror and type of supply/service that the Prime will provide. Then list each of the intended SB subcontractor and principal supply/services to be provided by the SB concern. Include the Unique Entity Identifier (UEI) and/or Commercial and Government Entity (CAGE) code for both the Prime and each listed subcontractor. A l s o P rovide the anticipated North American Industry Classification System (NAICS) code(s) that the Prime believes best describes the supply/service being acquired from each of the named subcontractors. SB subcontractors that qualify in multiple SB socioeconomic categories, as previously mentioned, should be listed under each applicable small business category.

Example: If a SB concern qualifies as a WOSB and a SDVOSB, the Offeror would add them to each category below in which they qualify – SB, WOSB, VOSB and SDVOSB.

(a) Prime Offeror:

Name of Company Identified (include UEI and/or CAGE Code)

Type of Supply/Services (include NAICS Code for Subcontractor)

Note, adjust/modify format for information box, if applicable.

(b) SB Concerns

(c) SDB Concerns (including ANCs and Indian Tribes)

Name of Company Identified (include UEI and/or CAGE Code)

(d) WOSB Concerns

(e) HUBZone Concerns

(f) VOSB Concerns

(g) SDVOSB Concerns

Note: Pursuant to the Small Business Act, Section 8(d), a business is considered small for Government procurement if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes supply/service being acquired. In other words, the size of the prime’s supplier is determined by the applicable NAICS code of their joint-venture, teaming, partnership, or subcontract, which may or may not be the same NAICS code as the one for the prime contract with the Government.

4. Describe the extent of commitment to use small business concerns (e.g., what types of commitments and of what duration, if any, are in place for this specific acquisition (written contract, verbal, enforceable, non-enforceable, joint- venturing, mentor-protégé, etc.).

5. When the type of commitment noted is a written commitment, provide copies of written commitment agreements.

Copies of such agreement will not count against the proposal page limitation.

Company” Type of Commitment (include duration):

6. Small Business Past Performance – Submit past performance information reflecting the Offeror’s past success in meeting the small business commitment for each project listed by the Offeror under Factor 1, Past Performance. If the project was performed for a Federal agency, all Individual Subcontract Reports (ISRs) and Summary Subcontract Reports (SSRs) from the Electronic Subcontracting Reporting System (eSRS) for the project must be submitted. If not performed for a Federal agency, for each project submitted under Factor 1, Past Performance, please submit the following:

(a) Name of the project;

(b) Total dollar amount and percentage of the contract value self-performed;

(c) Total dollar amount and percentage of the contract value subcontracted;

(d) Total dollar amount and percentage of the contract value subcontracted to small businesses.

(e) Total dollar amount and percentage of the contract value subcontracted to other than small businesses.

In addition to the above project submission information, the Government reserves the right to review Offerors Individual Subcontract Reports (ISRs) and Summary Subcontract Reports (SSRs) from the Electronic Subcontracting Reporting System (eSRS).
