1. Is file "Attachment 3 Price_Matrix_Template.xls required to be submitted? It is not listed in Volume III instructions as required to be submitted.

ANSWER. Yes, the spreadsheet (Attachment 3) is required.

2. If yes to above, please confirm that we can add formulas, rows (for multiple labor categories) to the pricing spreadsheet in each tab.

ANSWER: Yes, formulas and rows can add to the spreadsheet.

3. Where does an offeror put bid price for the following: Supplies, Materials, Equipment and Subcontractor costs?

ANSWER: Bid for Supplies, Materials, Equipment and Subcontractor costs are listed on the spreadsheet Attachment 3: (G&A expenses include building expenses, salaries and wages, insurance, licenses and fees and supplies; Overhead: include rent and utilities, business licenses, accounting fees, advertising expenses, and payroll and Subcontractor

4. Since there are no formulas in each tabs worksheet and no formulas in summary, what should we populate in calculated cells? Asking this to avoid being considered non-responsive if I change the spreadsheet to add totals.

ANSWER: Contractor to provide the formulas in each tab of the worksheet to provide their proposal totals.

5. In amendment 0002 on page 6 of 6, the government displays a direct labor table entitled "Attachment 3". Please advise as to how this table should be filled out because it does not match Attachment 3 Pricing Matrix columns. For example, Direct Labor Table has a column for Area Wage Determination Code and "Attachment 3 Pricing Matrix does not contain this column.

ANSWER: The spreadsheet listed in the solicitation was an example. However, due to several questions from contractors, the Government provided Attachment 3 for all contractor to use and submit with the proposal.
