APPLICABILITY QUESTION: PROVIDE RESPONSE IN COLUMN CFY vs CY2013Mid YearEnd of Year
1Clean Air Act (CAA) Emissions
Did any field activity locations maintain and report an inventory of actual air emissions from stationary sources to meet CAA requirements (e.g., Major Source with Title V permit) during the reporting period?

Note: If your field activity covers several locations (e.g., DAPS,DDC, DESC, DNSC, DRMS), provide data for each location reporting actual emissions. Provide a Yes or No Response Air Emissions CY 2012- January 1 - December 31, 2012 CY 2012 Previous CY

2Water Pollution Control (WPC) Permit Compliance
Do any field activity locations have WPC Permits?

WPC Permits are an authorization, license, or equivalent control document issued by the Environmental Protection Agency or a State/local agency to control water discharges to waters of the U.S. or host nation, or by a public sewage/wastewater treatment plant (Publicly Owned Treatment Works [POTW]). WPCPs include permits for domestic wastewater, industrial wastewater, and storm water discharges. WPCPs exclude storm water construction permits. Provide a Yes or No Response WPC Permits CY10

andCY 2012-January 1 - December 31, 2012CY2012Previous CY
When establishing the number of permits/facilities, use the following guidance:

▪ For the United States and its territories, any permit/facility subject to water pollution program requirements pursuant to the Clean Water Act, or state/local programs, excluding storm water construction permits.

▪ Overseas activities include facilities for which criteria are established under host-nation permits or Final Governing Standards.

▪ For overseas activities holding host-nation permits, report compliance based on discharges regulated by each permit and its requirements.

▪ For overseas activities without host-nation permits (FGS regulated Activity), combine all domestic, industrial, and storm water discharges and report as one regulated facility. WPC Permits CY11-1 CY 2013-1 January 1 - June 30, 2013 CY 2013 1 First Half CY

3Safe Drinking Water (SDW) Population
ADoes the water supply system have at least 15 service connections or regularly serve an average of at least 25 individuals daily at least 60 days of the year?Provide a Yes or No Response
BIs the public water [supply] system (PWS) located on a fixed installation (location with permanently constructed [non-mobile] facilities) controlled by DOD?
PWS includes any collection, treatment, storage, and distribution facilities under DOD control, and any non-DOD controlled collection or pretreatment storage facilities that are used primarily in connection with DOD-controlled systems. For example, if DOD controls the distribution system, the PWS is a DOD PWS.Provide a Yes or No Response
CIs the DOD PWS unregulation?

A DOD PWS is unregulated if it meets all of the following criteria:

(1) Receives its water from another regulated PWS (customer or privatized),

(2) Consists only of distribution and storage facilities (no collection or treatment),

(3) Does not sell any water, and

(4) Does not supply water to a carrier that conveys passengers in interstate commerce. Provide a Yes or No Response SDW Pop CY10

Note the following definitions:
Solid waste is refuse, garbage, scrap, sludge, and discarded waste that is routinely landfilled or incinerated. The waste is generally non-hazardous but may contain household hazardous waste, both hazardous and non-hazardous. Solid waste does not include construction and demolition waste, lead acid batteries, ethylene blycol based antifreeze, and used motor oil.
Construction and demolition (C&D) debris is material produced during construction, renovation, demolition, or deconstruction of residential and commercial buildings and their infrastructure. C&D typically includes concrete, wood, metals, gypsum wall board, asphalt, and roofing material.
Disposal is waste going to a landfill or incinerator (i.e., disposal facility).
Diversion is avoiding solid waste entering a disposal facility, including composting.
Installation population is the number of military and civilian personnel working or living at a location. For personnel living at a location, the population includes their families.
Costs exclude costs for disposal of lead acid batteries, ethylene glycol based antifreeze, and used motor oil.
Actual cost is the cost to manage your solid waste management program, which handles both disposal and diversion. The cost includes collection, transportation, disposal, diversion, and proceeds from diversion.
Disposal tipping fee is the receiving charge that a disposal facility charges to cover their expenses. Default is $40/ton. Include your disposal tipping fee if you know it.
Diversion cost is solely attibutable to the processing and marketing of material destined for diversion excluding collection and transporation; and including labor, maintenance, and other operational expenses for diversion.
Gross diversion proceeds are the income/earnings from the sale of diverted material.
