1. Prices must be submitted on all individual items of this Pricing Schedule.
Failure to do so may cause the proposal to be determined "unacceptable".
2. If a modification to a price is submitted which provides for a lump sum adjustment to the total price, the application of the lump sum adjustment to each item in the Pricing Schedule must be stated. If it is not stated, the offeror agrees that the lump sum adjustment shall be applied on a pro rata basis to every item in the Pricing Schedule.
3. The offeror shall distribute his indirect costs (overhead, profit, bond, etc.) over all the items in the Pricing Schedule. The Government will review all submitted Pricing Schedules for any unbalancing of the items. Any submitted Pricing Schedule determined to be unbalanced may cause the proposal to be determined "unacceptable".

(J) Supplemental Construction Considerations (NOTICE TO CONTRACTORS)
Referenced below are Supplemental Construction Considerations (clarifications) related to demolition of buildings; 732, 827, 1537, 1911 & 2201. This information is partially a result of questions raised, during a second site visit, held on 26 Jun 09. If there are any conflicts in information provided, Amendment 0005 supercedes any previous information provided.
All buildings will have a basic '5-foot Building Line' demolition limit (contractor shall saw cut all paving, concrete or asphalt concrete, at the 5-foot limit prior to removal) for extent of work to be performed, unless identified differently in features identified below:
1. Building 732 (Pricing Schedule referenced under CLIN 1014);
- Building, foundation slab and pavements are to be demolished/removed, up to the 5-foot building line.
- All utilities are expected to be capped at the 5-foot building line.
Any overheads are expected to be taken out to the nearest connecting pole, beyond the 5-foot building line.
- The existing fencing, outlying buildings/equipment/materials (anything beyond the 5-foot building line) are not part of this contract and are to be left intact.
- Building occupants, equipment and materials are expected to be removed by the government, prior to demolition. However, expectation is that this OPTION may not be exercised until up to the maximum; 90 Calendar Days (CD) duration, after issuance of Notice to Proceed (NTP). This is to allow complete building occupant/equipment/material removal, by the government.
2. Building 827 (Pricing Schedule referenced under CLIN 1015);
- Building, foundation slab and pavements are to be demolished/removed, up to the 5-foot building line. Exception being the concrete slab on the south side (exceeds the 5-foot building line limit), which is to be demolished/removed entirely, as part of the building demolition.
- All utilities are expected to be capped at the 5-foot building line.
Any overheads are expected to be taken out to the nearest connecting pole, beyond the 5-foot building line.
- Existing fence is not part of this contract and is to be left intact (government will remove).
3. Building 1532 (Pricing Schedule referenced under CLIN 1017);

4. Building 1911 (Pricing Schedule referenced under CLIN 1018);
- Building, foundation slab and pavements are to be demolished/removed, up to the 5-foot building line. Exception being a sidewalk being wider then 5-foot limit, this sidewalk is to be demolished/removed entirely, as part of the building demolition.
- A Loading Dock on the south side is not part of this contract and is to be left intact.
- A Transformer on the west side is not part of this contract and is to be left intact.
- A Storage Building on the west side is not part of this contract and is to be left intact.

A.4.7 SPECIFICATIONS
This list below replaces the one provided in Section 01 10 10 paragraph 4.7. The Contractor shall edit and submit the following UFGS specification sections, at a minimum, plus any other necessary and applicable Sections:
31 00 00 Earthwork 31 11 00 Clearing and Grubbing 31 31 16 Soil Treatment for Subterranean Termite Control
32 11 23 Aggregate Base Course (omit “GCA” requirements) 32 12 17 Hot-Mix Bituminous Pavement (in place of 32 12 16 as specified in paragraph 5.5 “SPECIFICATIONS”)
The following paragraph shall replace UFGS paragraph “TESTING” and related subparagraph “Check Tests on In-Place Densities” in the Contractor’s final edited specification Section 31 00 00, and this new paragraph “TESTING” shall be referenced in or copied to the final edited specification Section 32 11 23 paragraph “Field Density Tests”.
3.xx TESTING
Perform testing by a Corps validated commercial testing laboratory meeting the requirements of SECTION 01 45 04.00 CONTRACTOR QUALITY CONTROL and approved by the Contracting Officer. Determine field in-place density in accordance with the sand cone method ASTM D 1556, except the nuclear gauge method ASTM D 6938 may be used, as further qualified hereinafter, to determine in-place density of materials verified to be free of mica. When ASTM D 6938 is used, check the calibration curves as described in ASTM D 6938 and adjust using only
APPENDIX A – Page 5 the sand cone method as described in ASTM D 1556; check the calibration of the density gauge prior to the first use of each different type of material encountered and at intervals as directed by the Contracting Officer. Both ASTM D 1556 and ASTM D 6938 result in a wet unit weight of soil and when using either of these methods, use only method ASTM D 2216 to determine the moisture content for calculating in-place dry density of the soil. For a rough estimate of in-place density to control field activities only, the Contractor may perform moisture content testing by method ASTM D 4643 or ASTM D 4959 in conjunction with density testing by method ASTM D 6938. Density results determined by sand cone method ASTM D 1556 shall govern, and shall be used to calibrate (adjust) the results determined for similar materials by nuclear gauge method ASTM D 6938. If differing results are consistently determined, use of the nuclear gauge shall be discontinued and only method ASTM D 1556 shall be used. When test results indicate, as determined by the Contracting Officer, that compaction is not as specified, remove the material, replace it, and recompact to meet specification requirements. Perform tests on recompacted areas to determine conformance with specification requirements. For all tests required by these specifications, appoint a registered professional civil engineer to certify inspections and test results. These certifications shall state that the tests and observations were performed by or under the direct supervision of the engineer and that the results are representative of the materials or conditions being certified by the tests. The following number of tests, if performed at the appropriate time, will be the minimum acceptable for each type operation.
3.xx.x Check Tests on In-Place Densities
If nuclear gauge method ASTM D 6938 is used, check in-place densities by the sand cone method ASTM D 1556 at a ratio of one sand cone test for every six nuclear gauge method tests. Use these sand cone "check test" results to calibrate (adjust) the nuclear gauge results for representative materials as described in paragraph "TESTING" above.
The following paragraph shall replace UFGS paragraph “MAINTENANCE” in the Contractor’s final edited specification Section 32 11 23:
3.x MAINTENANCE
The base course shall be maintained in a satisfactory condition including preservation of moisture and density until the full pavement section is completed and accepted. Maintenance shall include immediate repairs to any defects and shall be repeated as often as necessary to keep the area intact. Any base course that is not paved over within 7 days of completion or that has received measurable rainfall prior to paving, shall be retested to verify that it still complies with the requirements of this specification. Any area of base course that is damaged or out of compliance shall be reworked or replaced as necessary to comply with this specification. The Contractor may elect to help preserve the base course by applying a prime coat meeting local air quality standards, but such application will not relieve the Contractor from compliance with the maintenance, verification, and re-work requirements specified herein.

A.4.8 PRELIMINARY DATA
A.4.8.1 Topography: Hill Air Force Base is located approximately 40 km (25 miles) north of Salt Lake City, just south of the Weber River and the City of Ogden, and northeast of the Great Salt Lake and the City of Clearfield. The Base is located on the Ogden, Kaysville, Roy, and Clearfield, Utah, 1:24,000 USGS 7.5 minute Quadrangles. Hill Air Force Base is situated on a large plateau approximately 60 meters (200 feet) above the surrounding area. The Base has MLLW elevations that range from 1,463 meters (4,800 feet) near the east side of the eastern airfield to 1,402 meters (4,600 feet) on the western boundary of the Base. The project site is located on Mitchell Lane between Balmer and Weiner, directly north of Building 472 on relatively flat terrain in the southern portion of Hill Air Force Base.
A.4.8.2 Climate:
Surrounding Climate Temperate and Sub humid Temperature Ranges -8º F to 98º F Average annual rainfall 21 inches Average annual snowfall 92 inches Design snow load 40 psf Base standard design freeze depth 73 inches Required domestic water pipe cover 4 feet Required fire protection line cover 5-1/2 feet Basic design wind speed 90 mph Table 1: Hill Air Force Base Climate Information
A.4.8.3 Areal and Regional Geology: The Base soil materials are composed of sand and gravel alluvial deposits interbedded with sedimentary lacustrine deposits of clay and silt. The alluvial deposits are thought to be from the erosion of the Wasatch Mountain Range and the lacustrine deposits are from ancient Lake Bonneville. Local areas of perched groundwater may be found on the Base, especially during the wet seasons and the spring thaw. Typically, aquifers are found at depths of 73 to 274 meters (240 to 900 feet) and the water wells at Hill Air Force Base are drilled to depths of 244 to 274 meters (800 to 900 feet) below the ground elevation.
A.4.8.4 Seismic Information: Seismic data were derived from seismic studies for Red Butte and Little Dell Dams located in the vicinity of Salt Lake City.
Salt Lake City is located near the center of the Intermountain Seismic Belt near the Wasatch Fault Zone. The area has the following earthquake magnitudes, accelerations, and recurrence intervals:
Status Magnitude Acceleration Recurrence Maximum 7.25 0.6 g 500 years
Operational 6.5 0.4 g 100 years Damaging 5.5 to 6.5 Not given 15 years minimum
Table 2: Seismic data for the Salt Lake City Region
From the 2006 International Building Code, the building area may be a stiff
APPENDIX A – Page 7 soil profile Site Class “D” with SPT “N” values of between 15 and 50, or a soft soil profile Site Class “E” with “N” values of less than 15. Seismic acceleration parameters for Class “B” (rock) sites can be found in UFC 3-310- 01, Table C-2. Although studies of the area have stated that Hill Air Force Base has a very low potential for liquefaction, loose sand/silt deposits may be present in the vicinity of this building site and if these become saturated, liquefaction is possible during seismic events.
A.4.8.5 Available Soils Data: Selected exploration logs and available laboratory data are provided in Appendix J, attachment J1 and J2. These include, respectively, 1) an April 1972 SPT boring investigation of the nearby NCO Open Mess (Bldg. 450) facility located northeast of the proposed CDC project site; and 2) a May 1982 SPT boring investigation of the nearby Religious Education Center facility located several hundred feet easterly of the proposed CDC project site. Both of these investigations were conducted by the Sacramento District Corps of Engineers, Geotechnical Branch, and these data represent the closest archival work found in USACE Sacramento files. The predominant subsurface materials encountered at both sites were described as loose medium-fine silty sand and sand with silt in the upper several feet, underlain by loose to firm medium-fine sand with traces of fine gravel. None of the explorations reported encountering free-standing groundwater to the maximum explored depth of 16 feet. Laboratory data provided in Appendix J, attachment J1 indicate soil moisture content between 5 and 8 percent. Native soils at Hill AFB are typically mildly to moderately corrosive, as verified by the laboratory testing for the NCO Open Mess project; therefore, cathodic protection is required for metallic utilities and other buried features.
Native soils generally contain negligible soluble sulfates and chlorides;
therefore, no special Portland Cement is required. Structural and exterior concrete except curbs and gutters is normally specified to have a 4,000 psi minimum 28-day compressive strength.
A.4.8.6 Preliminary Design Parameters (for cost estimation purposes):
